Should your project change, you must notify the NCDWR and submit a new application. If the property is sold, the new owner must Ge given a copy of ihis Czrtificaiion and approval letter, and is ihereby responsible for compiying with all the conditions. If total wetland fills for this project (now or in the future) exceed one acre, or of total impacts to streams (now or in the future) exceed 150 linear feet, compensatory mitigation may be required as described in 15A NCAC 2H .0506 (h) (6) and (7). Additional buffer impacts may require compensatory mitigation a�� described in l5A NCt�C 2B.0233. The post-construction removal of any temporary bridge structures must return the project site to its preconstruction contoi:rs and elevatians. The impacted areas shall be revegetated witk appropriate native species. �15A NCAC �J2H .C506(L)(2) 2. As a condition of this 401 Water Quality Certification, the bridge demolition and construction must be accomplished in strict compliance with the most recent version of NCDOT's Best Management Practices for Constniction and lViaintenance Activities. [15A NGAC 02H .0507(d)(2) �nd 15A NCAC 02H .05051b)(5)] 3. Bridge deck d:ains shall not �ischarge directly int� the stream. Stormwater shall be d;'rected across the bridge and pre-treated through site-appropriate means (grassed swales, pre-formed scour boles, vegetated buffers, etc.) before entering the stream. To meet the requirements of NCDOT's NPDES permit NCS0000250, please refer to the most � recent version of the North Carolina Department of Transportation Stormwater Best Management Practices Toolbox manual for approved measures. [15A NCAC 02H .0507(d)(2) and 15A NCAC 02H .0506(b)(5)] 4. Bridge piles and bents shall be constructed using driven piles (hammer or vibratory) or drilled shaft construction methods. More specifically, jetting or other methods of pile driving are prohibited without prior written approval from the NCDWR first. [15A N�AC 02HA�06(b)(2)] 5. This condition can be waived with prior approval from the NCD`vVR. [15A NCAC 02H .4506('u)(3} 7. All bridge construction shall be performed from the existing bridge, temporary work bridges, temporary cause��vays, er floa±ing or sunken barg�s. If work condit:ons req_uire barges, they �hall be floated into positio.i and then sunk.. The barges shall not be sunk and then dragged into position. Under no circumstances should barges be dragged along the bottom of the surface water. �15A NCAC 02H .0506(b)(3) 8. All stormwater runoff shall be directed as sheetflow through stream buffers at non-erosive velocities, unless otherwise apnroved by this certifica±ion. [15A NCAC 2B.0233] 9. All riparian buffers impacted by the placement of temporary fill or clearing activities shali be restored to the preconstruction contot�rs and revegetated. Maintained buffers shall be permanently revegetated with non-woody specizs �y the end of the grawi�zg season following �orr�pletion of const�uction. For t�:e pu�csE cf th:s condition, maintained buffer areas are defined as.areas within the iransportation corridor that will be subject to regular NGDOT maintenance activities including mowing. Approved BMP measures from th� most current version ofNGDOT Constn:ction and Maintenance Activi±ies mznua] such as sandbags, rock berms, cofferdams and other diversion structures shall be used to prevent excavation in flowing water. Heavy equipment shall be operated from the banks rather than in the stream channel in order to minimize sedimentation and reduce the introduction of other pollutants into the stream. f 15A NCAC 02H.0506(b)(3)] 18. All mechanized equipment operated near surface waters must be regularly inspected and maintained to prevent contamination of stream waters from fuels, lubricants, hydraulic fluids, or other toxic materials. Should waste or bor:ow sites, or access roads ta waste or borrow sites, be located in wetlands or streams; compensatory mitigation will be required since that is a direct imuact Gom road construction activities. Erosion and seiiiment control p: actices must be i�-� full compliance ��ith all specifications governing the proper desian, installation and operation and maintenance of such Best Management Practices in order to protect surface waters - standards [15A NCAC 02H.0506(b)(3) and (c)(3): . a. The erosion and sediment control measures for the preject must be designed, installed; operated, and maintained in accordance with the most recent version of the North Carolina Sediment and Erosion Contf�ol Planning and Design Manual. _ _. ___ b.
