Dear Mr. Bailey and Mr. Wanucha,
The North Carolina Department of Transportation is scheduled to replace Bridge #137 with a new
bridge at the same location. I have included the Attachments G& L with a project description and
a set of the project plans. A stormwater management plan is also attached. Please review this
project for compliance by your Division.
We plan to begin construction as soon as possible. If further inforxnation is required, please

Project Description — The project consists of replacing Bridge #137 over Forrest Creek on SR
1550 (Edmund Latta Road) and improving roadway approacl�es. The existing 20' long X 20'
wide, timber deck on I-beams supported by timber caps, concrete encased timber piles at end and
timber bulk(ieads, single span bridge is structurally deficient and will be replaced with a new,
single span 40' L X 28' W, cored slab, single span bridge at the same location. An off-site detour
will be utilized during construction. The pxoject involves no wetland impacts (there are wetlands
adjacent to the project but no wetlands within the project boundaries), 46 linear feet of permanent

stream impacts (associated with bank stabilization) and 1,897 square feet of Neuse River Riparian
buffer impact (1,267 sq. ft. Zone 1 and 630 sq. ft. Zone 2) which are listed as "allowable" per the
Neuse River Riparian Buffer Rules Table of Uses. Please note that this project is immediately
adjacent to tl�e Farrest Creek Mitigation Bank and that the conservation areas are on both the
upstream and downstream ends of the project. This bridge replacement will not encroach upon
the Mitigation Bank property. There are no deck drains on the bridge; instead, stormwater runoff
will be captured a proposed drop inlets and discharged onto a riprap pad located outside Zone 2 at
non-erosive velocities. All other stormwater is discharged into pre-existing ditches at non-erosive
velocities. Robert Lepsic surveyed the stream within the project boundary for the presence of
preferred habitat for the Dwarf Wedgemussel (Alasmidonta heterodon) and determined that
moderate habitat existed, however, no mussels were obseived during the March 16, 2016 site
visit. Additionally, there are no NHP records of dwarf wedemussel occuning within 1 mile of the
project site. Further, these findings were submitted to Gary Jordan with USFWS and he concurred
with the "no effecY' deteimination. Habitat surveys for Michaux's Sumac and Smooth
Coneflower were conducted on June 28, 2016 and determined that there is habitat for the named
species, however, no specimens were present within the project area; therefore the biological
conclusion is "No effect, Habitat presenY'. The Bald Eagle is protected in every county in North
Carolina under the Bald and Gold Eagle Protection Act; however, no suitable feeding/water
source is located within one mile of the project study area to support Bald Eagle habitat. Also,
Mr. Gary Jordan (USFWS) recently released the Programmatic Conference Opinion (PCO) for
the recently listed Northern Long Eared Bat (NLEB). This opinion, states in part, that "... it is the
Service's co�sference opinion that NCDOT activities i�n eastent North Carolina (Divisions 1-8), as
proposed, are riot likely to jeopardize the continued existence of the NLEB. "
