1) Facility self-reported excess freeboard on July 1, 2015. This exceedance has continued through October 31, 2016.
2) To decrease freeboard in preparation for hurricane and to prevent breach of the berm, fields were irrigated which resulted in runoff on the following dates: 10/4/16, 10/5/16, 10/6/16, 10/7/16, 10/8/16,
10/9/16, 10/10/16, 10/11/16,10/12/16, 10/13/16,10/14/16, 10/15/16,10/17/16, and 10/18/16. Ponding was also noted on the following dates: 10/20/16, 10/24/16, 10/25/16, and 10/26/16. Permission to pump
and haul to the City of Sanford's Big Buffalo Creek WWTP was received on 10/20/2016.
3) In efforts to decrease freeboard, the yearly application limit was exceeded for Field #5.
