Dear Mr. Meschel:
On September 18, 2007, the Division of Water Quality. (DWQ) received your application dated
September 14, 2007 (with Public Notice issued by the USACE on October 3, 2007, and received
by the DWQ on October 4, 2007), to fill or otherwise impact 1.75 acres of 404/wetland and
1,640 linear feet of stream (1,092 linear feet perennial stream and 548 linear feet intermittent
stream) to construct the proposed commercial development. On December 10, 2007, January 24,
2008, and April 30, 2008, the DWQ received additional information from you, however, the
DWQ has determined that your application remains incomplete and/or provided inaccurate
information as discussed below. The DWQ will require additional information in order to
process your application to impact protected wetlands and/or streams on the subject property.
Therefore, unless we receive five copies of the additional information requested below, we will
place this project on hold as incomplete until we receive this additional information. If we do
not receive the requested information, your project will be formally returned as incomplete.
Please provide the following information so that we may continue to review your project.
Additional Information Requested:
1. Please provide complete the Required Items checklists associated with every BMP
Supplement Form and provide ALL of the required items. Failure to provide complete
information in the next submittal will result in a return of application.
Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.

On September 18, 2007, the NC Division of Water Quality (DWQ) received your application for an Individual 401 Water Quality
Certification dated September 14, 2007 for impacts associated with the above referenced project. An Individual 404 Permit will be
required for this project. Please note that the following must be received prior to issuance of a 401 Water Quality Certification:
1. The 401 Certification cannot be processed until a copy of the Corps of Engineers Public Notice for this project is received at the
DWQ Central Office in Raleigh. No action is required on your part. The Corps of Engineers will forward the Public Notice to
this Office when it is published.
Until the information requested in this letter is provided, I will request (by copy of this letter) that the Corps of Engineers place this
project on hold. Also, this project will be placed on hold for our processing due to incomplete information (15A NCAC 2H .0507(h)).
