Dear Mr. Thomas,
We are requesting a Nationwide 14 permit for replacement of Bridge No. 188 with a
triple barrel box culvert on Shive Rd (SR 2564) over Second Creek (Class C) in Rowan
County. I have attached a Pre-Construction Notification application, a Vicinity, Soils, and
USGS Topographical map with the project area called out, the Stormwater Management
Plan, a'No Survey Required form for Historic Architecture', and a'No Archaeological
sites are present or Affected' for Archeology, and a Mitigation Acceptance Letter from
NC Division of Mitigation Services (NCDMS). Also attached are the Permit Drawings
and the Culvert Survey Report. The project will permanently impact 141 linear feet (53'-
culvert, 88'-bank stabilization) and temporarily impact 61 linear feet (dewatering) of
Second Creek. Low stream flow will be isolated into the center barrel, with 1' sills, and
the culverts will be buried 1' below the stream bed to ensure aquatic passage. The outer
high flow barrels will have 2' sills and floodplain benches constructed at the inlet and
outlet of the culverts. The low flow barrel will be backfilled with material that is excavated
from the stream or floodplain and may be supplemented with rip rap if needed. The high
flow barrel will be backfilled with rip rap or native bed material of appropriate size to the
sill elevation in order to expedite the establishment of a natural stream bed. The site for
the installation of the culverts will be dewatered during construction in accordance with
NCDOT's Best Management Practices. A temporary overhead power line will be
installed over the upstream waters during construction. The power line will be located
approximately 40 feet upstream of the proposed culvert inlet. No additional impacts to
Waters of the US are proposed as a result of the utility installation.
The USFWS lists Schweinitz's sunflower (Helianthus schweinitzii) as endangered and
Northern Long-Eared Bat (NLEB-Myotis septentrionalis) as threatened for Rowan
County. Due to habitat in the project area for Schweinitz's sunflower, a survey was
conducted on November 12, 2014 to rule out the presence of these individuals within the
project area. No Schweinitz's sunflower individuals were found. Therefore; it was

The existing site is a bridge. The surrounding land is a mixture of agricultural, rural residential, and forested areas.

3d. Explain the purpose of the proposed project:
Replacing a structurally deficient single span bridge with (3) 10' X 8' RCBC's on a 120 degree skew.
3e. Describe the overall project in detail, including the type of equipment to be used:
Traffic will be detoured off-site and erosion and sedimentation control measures will be installed. Water will be diverted
around the construction area and the old bridge will be removed. The proposed structure will be replaced on its existing
alignment and a new culvert structure (3) 10' X 8' RCBC will be cast in place. The channel will then be excavated and
realigned for low flow shaping, and a floodplain bench with stabilization will be established on each bank. Water will be
pumped around the work area during construction by installed impervious dikes upstream and downstream of the
existing bridge. Pumped water between dikes will be directed into a special stilling basin. Equipment to be used includes
a track hoe, dump truck, paving equipment, pumps, and various hand tools.

1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
The proposed project will remove and replace a structurally deficient bridge and replace it with a triple box culvert.
Roadway approach work is minimized as much as is practical to reduce the overall project footprint. An off-site detour will
be employed to avoid the need for a temporary parallel structure. The culvert slope is 0.94 %, so native bed material shall
be placed between the sills in the center culvert to provide a continuous low flow channel. Further justification is provided
in the stormwater management plan. Bank stabilization will be minimized to those areas where deemed necessary.

Erosion and sedimentation BMPs will be installed prior to construction. Water will be diverted around the work area to
prevent sedimentation of downstream aquatic resources. Impacts will be minimized by strict enforcement of Best
Management Practices for the protection of surface waters, restrictions against the staging of equipment in or adjacent to
waters of the US and coordination (including a pre-construction meeting) with the Division Environmental Supervisor.

Project Description:
This project is an intensive archaeological survey and evaluation for the replacement of Bridge No. 188
on SR 2564 (Shive Road) over Second Creek in Rowan County. As defined by the NCDOT, the Area of
Potential Effects (APE) for archaeology measures approximately 300 feet (91.44 m) from either end of
Bridge No. 188. The corridor is approximately 150 feet (45. 72 m) wide, extending 75 feet (22.86 m) on
each side of SR 2564 from its present centerline.
SUMMARY OF ARCHAEOLOGICAL FINDINGS
The North Carolina Department of Transportation (NCDOT) Archaeology Group reviewed
the subject project and determined.•

An archaeological survey and evaluation of the proposed replacement of Bridge No. 188 on SR 2564
(Shive Road) over Second Creek in Rowan County was conducted on February 5, 2015, by Terri Russ
and Laurie Griesmer of Environmental Services, Inc. (ESI). As a result of the investigations, one
prehistoric isolated artifact find (31RW262) was located within the project's APE. This archaeological
site was recommended not eligible for the National Register; no further archaeological investigations are
needed for this project. I concur with this recommendation as the proposed bridge replacement project
will not impact significant archaeological resources. If the project expands and impacts subsurface areas
beyond the deiined APE, further archaeological consultations will be necessary.

Brief description of review activities, results of review, and conclusions:
ESI conducted an intensive archaeological survey for the replacement of Bridge No. 188 over Second
Creek on SR 2564 (Shive Road) in Salisbury, Rowan County, North Carolina (Figures 1 and 2). As
defined by the NCDOT, the survey area (Area of Potential Effects [APE]) for archaeology measures
approximately 300 feet (91.44 m) from either end of Bridge No. 188. The corridor is approximately 150
feet (45.72 m) wide, extending 75 feet (22.86 m) on each side of SR 2564 from its present centerline.
A map review and site file search conducted by NCDOT on 1 December 2014 revealed that no previously
recarded archaeological sites have been identified within a one mile radius of the APE. A search of the
North Carolina State Historic Preservation Office online database (HPOWEB GIS Service) revealed no
previously recorded historic architectural resources within the APE that have the potential to yield intact
archaeological deposits.
Topographic maps, aerial photography, United States Department of Agriculture (USDA) soil survey
maps, and historic maps were examined for information on natural or cultural factors that might have
affected site location or preservation. The c. 1910 Rural Delivery Routes map shows what appears to be
the current road alignment and crossing. The 1914 Soil Survey map for Rowan County also shows a road
or crossing in the vicinity of the current bridge. Later maps, including the 1938 and 1953 Rowan County
Highway maps, show no apparent changes to the road alignment or bridge. No structures are depicted
within the APE.
The APE consists of wooded areas, overgrown fallow fields, and a residential driveway and lawn. Bridge
No. 188 and Shive Road cross Second Creek from the northwest to southeast. Second Creek flows to the
east and north east into High Rock Lake, part of the Yadkin-Pee Dee River Basin. Map units (soil series)
are named for the major soil or soils within the unit, but may have minor inclusions of other soils (NRCS
2013). The three soil units mapped within the APE are listed in Table 1. The floodplain of the APE is
composed of Chewacla loam (ChA), a nearly level, somewhat poorly drained soil series. The side slopes
and terraces are mapped as well drained Enon fine sandy loam (EnB; 2-8% slopes) and Mecklenburg clay
loam (MeB2; 2-8% slopes). Mecklenburg clay loam is a moderately eroded soil with a typical profile (if
intact) of less than 20 centimeters of clay loam over clay subsoil.

The current archaeological investigation included pedestrian (visual) inspection and shovel testing within
the APE. Photographs of the project area are shown as Figures 3-8. Surface visibility was generally
poor due to ground cover and existing vegetation; however, a systematic visual inspection of the APE was
undertaken to search for surface artifacts, above-ground structural remnants, or other signs of cultural
activity. No cultural materials or above ground remains were observed. All excavated shovel tests
measured 30 centimeters in diameter and were excavated to sterile subsoil or the water table. Soils were
excavated by natural levels (soil strata) and screened through a 0.25-inch hardware mesh.
The northeastern quadrant of the APE consisted of a fallow, overgrown field. Three shovel tests (STs 55—
57) were excavated in this portion of the project area. ST 55 revealed 25 centimeters of brown sandy
loam (plowzone) over brown and grayish brown mottled silty clay loam. The water table was

encountered at 35 centimeters below surface. STs 56 and 57 encountered similarly poorly drained soils,
with wet silty clay loam encountered at 40 and 50 centimeters below surface, respectively.
The southeastern quadrant of the APE consisted of a partially wooded area and fallow field. Three shovel
tests (STs 58-60) were excavated in this portion of the project area. ST 58 encountered 15 centimeters of
dark grayish brown sandy loam over reddish brown loamy clay subsoil. ST 59 encountered 20
centimeters of dark brown sandy loam over a five centimeter thick layer of gravel. Subsoil was a dark
yellowish brown silty clay loam (encountered at 25 centimeters below surface). ST 60, excavated near
the western edge of the field, encountered 30 centimeters of disturbed mottled brown and dark brown
sandy loam and gravel over a grayish brown and light brown mottled silty clay loam. Water was visible
at 45 centimeters below surface.
The southwestern quadrant of the APE was largely wooded, with a small, recently clearcut field along the
north side of Second Creek. STs 61-63 were excavated in this portion of the project area. STs 61 and 62
encountered 25 centimeters of grayish brown sandy loam over dark yellowish brown silty clay loam.
From 25 to 45 centimeters below surface, the silty clay loam transitioned to a compact loamy clay. The
excavation of ST 63, located along the tree line of an adjacent iield, yielded a 15 centimeter layer of dark
yellowish brown sandy loam over a yellow silty clay.
The northwestern portion of the APE consisted of a relatively level residential lawn and driveway gently
sloping toward Second Creek. Three transect shovel tests (STs 64-66) were excavated in this portion of
the project area. As a result, one new archaeological site, 31RW262, was recorded and is described
below.

Site Description: ST 64 was excavated along a level area east of a residential driveway within an existing
power line corridor at the edge of the APE. A single piece of inetavolcanic debitage was recovered from
the uppermost portion of Stratum I(between 0 and 15 centimeters below surface). Soils in the shovel test
consisted of 25 centimeters of brown loamy sand over 25 centimeters of light brown sandy loam. Subsoil
was a light yellowish brown sandy clay. Delineation shovel tests dug at 15-meter intervals around ST 64
did not encounter any additional cultural materials.
Recommendations: This prehistoric site consisted of one nan-diagnostic lithic artifact recovered from the
uppermost disturbed soils of a single shovel test. This isolated artifact find does not have the potential to
yield significant information pertaining to our knowledge of local and regional prehistory. This site is
recommended not eligible for the National Register, and no further wark is recommended at this location.

In summary, based on the results of the background research and field investigations, no National
Register eligible archaeological resources are located within the APE for Bridge No. 188 in Rowan
County. It is recommended that this project be allowed to proceed without concern for impacts to
significant cultural resources. Should the boundary of the proposed APE be expanded or moved,
additional archaeological investigations may be necessary, as determined in consultation with the
NCDOT and/or SHPO per the Programmatic Agreement.
