I was notified by DOT of the several issues described below that will need to be addressed via a permit modification.
Sediment losses and stream crossing incidents were reported to DWR immediately and were confirmed on site the next
day.

Comment: On the B Section, a iurisdictional stream at Site 3 was missed durinq drivewav construction on a side
slope. The stream was crossed multiple times bv aradinq equipment. DOT decided to modifv the permit
to relocate a drivewav and return the channel to its oriqinal profile. Modification is pendinq. A linear
wetland at Site 5 was filled with offsite sediment erosion due to a recent 3.5" storm event. DOT decided to
modifiv the permit to convert the wetland to a sedimentation basin. Once proiect is complete the basin
would be returned to its oriqinal dimensions. Modification is pendinq. On the A Section, Site 12 (and
others to comel will need to have riprap placed at the inlet of the culvert due to steep slope issues and to
prevent headcuttinq. A permit modification is pendinq for this.
