Box 3136 Greensboro, North Carolina 27402-3136 Dear Mr. Ingle: NC Department of Environmental Quality Received AUG 15 2016 Winston-Salem Regional Office Please reference the "Level 1 Stream Restoration Monitoring, Fifth Year Monitoring Report" documents, dated April 20, 2016 and July 8, 2016, sent by your office concerning the New Garden Road Widening, Phase II, Project P-3741, in Greensboro, Guilford County, North Carolina. The stream restoration project included the restoration/relocation and enhancement of 322 linear feet and 155 linear feet of stream channel, respectively, serving as compensatory mitigation for stream impacts associated with the replacement of an existing bridge with a bottomless arch culvert as well as a waterline replacement. These activities were authorized via Nationwide Permits 12/14/27/33 on April 5, 2010 by the US Army Corps of Engineers, Raleigh Regulatory Field Office (Corps). Note that Mr. David E. Bailey of the Corps and Ms. Sue Homewood of the North Carolina Division of Water Resources (NWDWR) visited the site on April 26, 2016 to conduct a mitigation compliance inspection in support of Year 5 -Closeout for this site. After review of the site and the above referenced documents, it was the determination of the Corps and NCDWR that this site adequately met the success criteria necessary with no remedial action required.
