Good afternoon team!

I've attached pictures of the above Bridge showing both the upstream and downstream ends. The original issues at this
bridge included some geotextile fabric left in the channel, 57 stone mixed in/on top of the rip rap at the wing walls,
unstable/non-matted banks, and an area within the stream (upstream end) where there appeared to be a large area of
sediment/57 stone (possibly left from the pump around operation during the culvert installation).

I've attached a quick word document that has the "before" and "after" shots of the creek/culvert crossing (just by way of
reference). I've also included a few other pictures here for your review.

Again, my apologies for dropping the ball on photo documentation. Let me know if you have any questions or additional
concerns.

Yes, the repair work was done. I think I dropped the ball on this one. This was about the same time my cell phone ate

Hi Nikki. Thanks for the info. One of the completion certificates jogged my memory. Bridge 151 on SR 2307 (SAW-2014-
001079) was found to have a few compliance issues during our August field visit last year (see attached
correspondence). I couldn't find confirmation that the issues were corrected and resolved. Can you provide me the
documentation that the compliance work has been completed? Pictures post correction would be most helpful.
