Monthly monitoring is required for the Fayetteville facility as per the Tiered program requirements in Part II, Section B of the facility permit for FV-002 and FV-006. For the monthly sample period monitoring, a sample was taken on June 24,2016 for FV-002 and for the semi-annual sample period monitoring,a sample was taken June 28,2016 for FV-006. FV-006 is a new Storm Water Outfall. The analytical results with associated parameters for this monthly(FV-002&FV-006) monitoring are included in the table below. The new manpower is focused on keeping roadways and other impervious surfaces that lead to FV002 clean on a daily basis. The plant also has contracted a consultant to evaluate and develop options for structural BMP's to lessen the nutrient load and suspended solid load in the storm water. Elevated results for COD,TSS,and pH were reported for FV006. After investigating,it was determined that the Environmental Pit(separation pit)had a sludge layer on the bottom of the pit. A contractor was hired to thoroughly clean all solids from the pit and the plant will continue to clean the pit to improve the quality of the effluent from FV006. I certify,under penalty of law,that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted.
