In accordance with Part III, E(2) of the above referenced NPDES stormwater permit, the original
and one copy of the monthly monitoring reports Year 1 — Period 2 are attached. The period for
sampling ends on December 31, 2015.
On 27-28 October and 2 November 2015, samples were collected from stormwater outfalls
SW001, SW002, SW003, SW008, SW009, SW011, SW012, SW015, SW016, SW018, and
SW020. The laboratory results were received on 13 November 2015. On 6 October 2015, an oil
and grease sample was collected at NPDES outfall 004 (same outfall as SW011) as part of the
quarterly sampled required by the Allen Steam Station NPDES wastewater permit
(NC0004979). Both stormwater and applicable wastewater results are included in this submittal.
Parameters that exceeded benchmark values are given in the table below. Duke Energy has
performed the required tier one steps for these outfalls.

In accordance with Part III, E(2) of the above referenced NPDES stormwater permit, the original

SW001, SW002, SW003, SW008, SW009, SW011, SW012, SW015, SW016, SW018, and
SW020. The laboratory results were received on 13 November 2015. On 6 October 2015, an oil

quarterly sampled required by the Allen Steam Station NPDES wastewater permit
(NC0004979). Both stormwater and applicable wastewater results are included in this submittal.
