Sullins, Director Division of Water Quality May 23, 2008 DWQ Project # 06-1798, Ver. 6 Johnston County CERTIFIED MAIL: RETURN RECEIPT REQUESTED Mr. Ronald Mikesh Summerwind Plantation, LLC 3061 Berks Way, Suite 202 Raleigh, NC 27614 Subject Property: Summerwind Plantation Ut to Swift Creek [030402, 27-43-(8), C, NSW] REQUEST FOR MORE INFORMATION Dear Mr. Mikesh: On February 12, 2008, the Division of Water Quality (DWQ) received your revised application dated February 8, 2008, to fill or otherwise impact 0.12 acres of 404/wetland, 165 linear feet of perennial stream, 230 linear feet of intermittent stream, 0.01 acres of open water impact (pond), and 14,324 square feet of Zone 1 Neuse River basin protected riparian buffers and 10,125 square feet of Zone 2 Neuse River basin protected riparian buffers to construct the proposed residential subdivision. On April 16, 2008, the DWQ received additional information from you, however, the DWQ has determined that your application remains incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will place this project on hold as incomplete until we receive this additional information. If we do not receive the requested information, your project will be formally returned as incomplete. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. Please provide the appropriate BMP Supplement Forms in an organized manner, with all parts of the form for each BMP attached together. Please also refer to the BMPs in a consistent manner thfoughout the submittal; for example, the same BMP is called "Phase 1B Stormwater Wetland" and "BMP #2" in different portions of the submittal. The DWQ is unable to conduct a thorough review due to the difficulty of finding the necessary information. 2. Please completely fill out the Required Items Checklists and provide ALL required items, including the planting plans. Please refer to plan and page sheet numbers that can be found in the submittal. Failure to provide this information in the resubmittal will result in a return of application. 3. In areas where regrading is proposed to provide appropriate filter strip slopes, you must show all of the new grading lines within and around the perimeter of the proposed filter strip. The filter strip downslope of level spreader 2 is directing a portion of the flow to the bypass channel. Please redesign so that the bypass channel is completely out of the filter strip. 5. BMP #4 is proposed to be a wet detention pond, however, no forebay has been provided. 6. All BMPs must be designed, at a minimum, to meet the standards in the NC BMP Manual, available at: http://h2o.enr.state.ne.us/su/bmp forms.htm. Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
