Please use the up to date Level Spreader Supplement Form on the DWQ web site
at: http://b2o.enr.state.nc.us/su/bmp fonns.htm. For ease of your design and the
DWQ's review, please fill the Design Summary form out digitally to benefit from
interactive features of the form.
2. Please completely fill out the Required Items Checklists for all three of the level
spreaders. Not all of the required information was included at the correct scales.
In particular, the DWQ noted the following deficiencies:
a. Plans of the entire site at a V'= 50' scale were not included.
b. The drainage areas of the areas draining to the level spreaders were not
depicted on the plan sheets.
c. A scaled section view of the level spreaders was not included on the plan
sheets. The copy of the detail from the BMP Manual is not sufficient.
d. Page numbers/plan sheet numbers were not included.
Level spreaders 1 and 3 are directing some of the flow toward the fill slopes for
the roadway embankment. Please relocate these level spreaders such that there
will not be channelization of flow in the filter strip.
