Dear Sir or Madam,
Please review the attached Pre-Construction Notification and supporting documentation. We are
seeking a Nationwide 12 permit and the accompanying DWQ General Certification for the minor
wetlands impact associated with the installation of 280 feet of sewer lines for the project shown.
All work is expected to be completed with the wetland area restored to its' original elevation
within one (1) week from the beginning of construction.
The walkways and residences will be elevated on piers to avoid wetland impacts.
Attached are seven (7) collated sets of all USACE forms with supporting documents, along with
our check in the amount of $200 for the review fee.
