The DWQ has determined that your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and will place this project on hold as incomplete until we receive this additional information. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. Please re-submit your site plans on full plan sheets at a scale of no smaller than 1 "=50' with topographic contours. 2. For any crossing(s) for which a culvert is proposed please provide cross section details showing the provisions for aquatic life passage. 3. Please provide the location of any proposed stormwater management practices as required by GC3705. 4. Please provide plans and profiles for all the stormwater management devices as required by GC3705. 5. Please indicate all stormwater inlets and outfalls on the site plan. Please provide an inventory of proposed project imperviousness area based on the estimated built-out conditions. Please include all proposed building footprints, roads, driveways, sidewalks, gravel-surfaced areas, amenity areas, etc. 7. Please provide building envelopes for all lots with wetlands, streams or buffers on the site plans. 8. Please indicate all stream impacts including all fill slopes, dissipaters, and bank stabilization on the site plan. 9. Please enumerate all riparian buffer impacts on the site plan and clearly label impacts (Buffer Impact No. 1, etc.). 10. Please locate all isolated or non-isolated wetlands, streams, and other waters of the State as overlays on the site plan. 11. Please locate all of the protected riparian buffers as overlays on the site plan, clearly showing Zone 1 and Zone 2. 12. Please indicate the diffuse flow provision measures on the site plan and submit diffuse flow provision measure worksheets and signed and a notarized Operations and Maintenance Agreement. 13. The lot fills on the plans do not appear to be necessary. Please eliminate them or provide additional information as to why they are necessary for this project. 14. This Office believes that the road crossing can be moved or reconfigured to minimize impacts to the stream. Please revise your plans to minimize impacts to the stream at the road crossing. 15. Please provide DWQ buffer determination and intermittentlperennial stream determinations for the site. Please include DWQ stream forms and upstream and downstream photographs. 16. Please culvert or provide additional information regarding your "road to nowhere" located in the northern portion of your site. 17. Please discuss when the pond was drained. Is it now a stream? Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
