Dear Mr. Piazza:
The Stormwater Division has reviewed the subject Stormwater Management plans received on
12/04/2007, and revised plans received on 01/17/2008. The Storm-water Division finds the plans
acceptable and the hereby issues this "Letter of Approval - APPROVED WITH MODIFICATIONS."
Required modifications are listed on the attached page- Enclosed are the Stormwater Management Permit
and a copy of the approved Stormwater Management Plan.
Please have your contractor notify the Stormwater Division, phone number (336-570-6616), when a
preeonstruction conference has been scheduled so that we may attend and discuss the Stormwater
Management Plan.
The City's Stormwater Ordinance is a performance-oriented program requiring protection of the natural
resources and adjoining properties. Following commencement of this project if it is determined that the
plan is not meeting the requirements of the ordinance, then this office will require that the plan be
modified.to comply with the City's ordinance. Also, please be advised that the ordinance requires that a
copy of the approved stormwater management plan be on file at the job site.
Per City Ordinance Appendix D, Section 5-503 notice is hereby given that agents, officials, or other
qualified persons authorized by the City will periodically inspect any land, building, structure or premises
to ensure compliance with the ordinances, or rules or orders adopted or issued pursuant to the ordinance,
and to determine whether the measures required in the Plan are effective in controlling stormwater runoff

resulting from development or redevelopment. Compliance with the City's Erosion and Sedimentdtion

federal agencies. This could include the Division of Water Quality under storm water or water quality
regulations; the U.S. Army Corps of Engineers under Article 404;jurisdiction; county, city or town `
agencies under other local ordinances; or other approvals that may be required. This approval cannot
supersede any other permit or approval, nor shall it exempt the applicant from obtaining other applipable
approvals from local, state, or federal authorities.

1. Provide a copy of the Operation and Maintenance Manual for the wet detention pond.
2. Provide a fixed vertical sediment depth marker for measurement of sediment deposition
on wet detention pond.
;. Provide dimensions on plan Detail 1/C7.24 for width (perpendicular to flow direction)
and height of "teeth" on energy dissipators (6-inch spacing and 6-inch depth parallel to
flow direction are given).
4. Any sediment accumulated during site construction in the wet detention pond, the storm
drainage piping system or conveyances, or in storm drainage structures shall be removed
and properly disposed of prior to final closeout of the stormwater permit.
NOTE: Be advised that the above "Modifications" are considered to be part of the approved
plan and part of the conditions of the stormwater permit.

Enclosed for your consideration are the following materials for the referenced project for coverage under Water
Quality Certification 4WGC 3704, under the non-express program.
• Review Fee (Check payable to N.C. Division of Water Quality in the amount of $570.00).
• Project PCN form and attachments (7 sets)
The application is for 295 linear feet of stream impact to an Un-Named Drainage tributary to Gunn (a.k.a. Gum.)
Creek in the Cape Fear River basin. The drainage feature was characterized as "unimportant" at a pre-
application meeting and field review held on November 20, 2007. Mr. Andy Williams and Ms Crystal
Amschler attended for COE and Ms. Sue Homewood attended for DWQ. Mr. Williams determined that
coverage would fall under NW 14 and that mitigation would not be required. A copy of the meeting notes is
included in the PCN package.
The project Stormwater Management Plan has been submitted) to the City of Burlington for review under the
NPDES Phase II program;: approval documentation will be forwarded separately. The final jurisdictional
delineation plats have been presented to Mr. Williams for signature.
By copy of this letter, we are forwarding a set of review materials to Ms. Homewood'.
