Subject: Request for Renewal of NW 3 Permit (200510843) and DWQ 401 (no written certification) for the
replacement of Bridge # 78 over the Little River on SR 1367 in Perquimans County.
The North Carolina Department of Transportation is requesting the renewal of NW 3 Permit #
(200510843) and DWQ certification (no written certification). This permit authorized the replacement of a
75 ft. bridge with a 110 ft. cored slab bridge on existing location. Due to scheduling delays this work was
not completed by the expiration date of March 18, 2007. NCDOT intends to begin this project within the
year. NCDOT also received a CAMA major permit (109-OS) for this project, which has an expiration date
of 12/31/2008. If you need any additional information or have any questions concerning this request please
