In accordance with yaur application received duly 3, 2a04, we arc foruvarding herewith Permit
No. WQ0018725, dated August 28, 20c~0, to New ~Ianover County for the eonstn~ction of the subject
wastewater collection system extension. This permit shaA be effective from the ,date o;f-issuance until
rescinded and shall lie subject to the Conditions and limitations as specified therein. This cover letter
shall be considered a part of this permit and xs therefore incorporated therein by reference.
Please pay particular attention to Permit Condition 2, which reQuixes that the wastewater
collection facilities be properly opexated and maintained in accordance with 15A NCAC 2I-i .0227 or
any indivxdu~l system-wide collection system permit issued to the Permittee. Flease note Permit

Permitting of this project does not constitute an acceptance of any part of the project that does
not meet 15A NCAC 2H .a2ap; the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted Febroaty 12, 1996 as applicable; atad fife Division's Minimum Design Criteria
for the Past-Track Permitt7ng of Pump Stations and PQrce Mains adapted June 1, 2000 as applicable,

Pen~,ittee's responsibility to ensure that the as-constructed project meets the appropriate design criteria
and roles. Failure to comply may result in penalties in accordance with North Carolina General Statute
§143~21S.6A through §143-215.60, construction of additional or replacement wastewater collection
facilities, and/or referral of the North Carolina-licensed ~'rofessional Engineer to the licensing board.
1<n accordance with the provisions of Article 21 of Chapter 143, General Statutes of North
Caxalina as amended, and other applicable Laws, ItuIes, and Regulations, permission is hereby granted
to TVew I~anover County for the construction of apprdaimately $x,990 linear feet of $-inch gravity sewer,
27,524 linear feet of 10-inch gravity sewer, and 9,11a linear feet of 13-inch gravity sewer; a 1,070-
gallon per minute pump station {No.l Buckeye) equipped with a hydrogen peroxide-based odor control
system, a 390-gallon per minute pump station (No.2 Falcon Poxrlt), a 230-gallon per minute um
station (No.3 Stoneybroak}, a 20a-gallon pex minute pump station (No.~F Caudell), a 18a-gall n per

The sewage and wastewater collected by this systdm shall be treated in the City of Wilmington
Wastewater Treatment Facility (NPDFS Permit No. NC0023965) prior to being discharged into the
receiving stream.

'T'his Permit sh$It be effective from the tiara of issuance until xescinded and shall be subject to the
follovying specked conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities axe constructed in
accordance with the conditions of this permit; 15A NCAC 2H .020p; the Division of 'Water
Quality's (Division} Gravity Sewer Minimum Design Criteria adapted' February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast~Track Permitting of Pump Stations
and force Mains adapted June 1, 20100 as applicable; and other supporting materials unless
specifically mentioned herein, -. _~
2. This permit shall be effective only with respect to the natuz~e and volume of wastes described in the
application and other supporting data.
3, The waStevvater collection facilities shall be properly maintained and operated at alI times, The
1'ermittee shall maintain compliance with an individual system-wide collection system permit for the
operation and maintenance of these facilities or with 16A NCAC 2~I.0227 as follows:
a. ~'he sewer system shall be affectively~maintained said operated at sit tunes to prevent.disc>aazge
to land or Surface waters, and any contravention of the groundwater standards in 1sA NCAC
2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed prior to January 1; 2004 and shall be actively
maintained.
- c. An operation and maintenance plan shall be developed and implemented.
d. Pump sta~ons that are not connected. to a telemetry system shall be inspected at least three
times per week until July 1, 2000; thereafter, pump stations sha11 be inspected at least daily, as
defined in 15A NCAC 2B .0503(5), until July 1, 2001; and thereafter, pump stations shall be
inspected every day. Bump stations that are connected w a telemetry system shall be itspected
at least once per week.
e. High-priority sewer lines shall be inspected at least once per every six-month period of time.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Inspectit~n and maintenance records shall be maintained for a period of at least three years.

accordance with 15A NCAC 2$ .QSOG(a}, and public native shall be provided as required by
North Carolina General Statute §143-z1$.1C.

4. This permit shall not be transferable. In the event there is a desire for the wastewater collection
facilities to change ownership, or there is a name change of the )?errnittee, a formal permit regties#
shalt be submitted to the Division accompanied by documentation from the parties involved, and
other supporting materials as may be appropriate. 'Y'he approval of this request shall be considered
an its merits and may or may not be approved.
5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the existing
utilities nor result in an overflow or bypass discharge of wastewater to the surface wags of the
State. -
6. Upon completion of construction and prior to operation of these permitted facilities, a certification,
a copy of the construction record drawings, as well as supporting design calculations for any ump
stations permitted as part of this project shall be received from a North Carolina-Iic~sed
Professional Engineer certifying that the facilities have been installed in accordance with this
pernut; the certificate of coverage; 1SA NCAC 2l~ ,0200; the Division's Gravity Sewer Design
Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for Che
Fast-Track Permitting of Fump Station and Force Main adopted June I,' 2000 as applicable; and
other supporting materials. If this project is to be completed in phases and partially certified, you
shall retain the responsibility to track further construction approved under .the same permit, and
shall provide a final certificate of completion once the entire project has been completed. A copy of
the construction record drawings, indicating the facilities constructed in the phase being certified,
shalt be submitted with each partial certification, Mail the Engineer's Certf'3cation, one copy of the
"Construction Record Drawings", and one copy of the supporting design calculations to the Non-
Discharge ]?ermitting Unit;' 1617 Mail Service Center, Raleigh, NC 27$94-],617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life
of the wastewater collection facilities.
$. Failure to abide by the conditions and limitations contained in this permit; the certificate of
coverage; IAA NCAC 2pT .0200; the Division's Gravity Sewer Design Criteria adapted February
12, 1996 as applicable; the Division's Nrnimum Design Criteria for the Past-Track Permitting of
Pump Station and Force Mains adopted June 1, 2000 as applicable; and other supporting materials
nzay subject the Permittee to an enforcement action by the Division, in accordance with North
Carolina General Statutes §143-215.6A through §143-215.$C.
9_ In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or replacement
facilities.
14. The issuance of this geaxnit shall not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances that maybe imposed by other govennrnent agencies (local, state and
federal) which have jurisdiction, including but not limited to applicable giver buffer rules in 15A
NCAC 2Y3 ,0200, erasion and sedimentation control requirements in l5A I~'CAC Ch. 4 and under
the Division's Creneral Permit NCCr0100000, and any requirements pertaining to wetlands under
15A NCAC 2l3 ,0200 and 15A NCAC 2FI.0500.
11. Noxtcompliatice Notification:
The Permittee shall report by telephone to the Wilmington regional Office, telephone number (910}
395-3900, as soon as possible, but in no case more than 24 hours ar an the next working day,
following the occurrence ar first knowledge of the occurrence of either of the fallowing:

a, Any process unit failure, due to known ox unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage oz breakage, etc.; or
b, ~„y failure of a purapzng station or Sewer line resulting in a by-pass directly to receiving waters
without treatment of all or any portion of the influent to such station or facility,
Persons reporting such occurrences by telephone shall also file a written xeport in letter fo~an within
five days following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur.
12. This pez~mit sha11 be for the construction of the wastewater collection facilities only. No flow shall
be made tributaxy to the subject facilities until permit modification application is submitted ~c-ith
the appropriate fee), a modified pertnit is issued, and the Engineer's Certification is mceived and
approved by the Division.
