I just got that request today and yes you are correct because the widening doesn't involve moving existing shoulders and ditches (there are no jurisdictional impacts from the pavement widening), the actual replacement of the culvert pipes will only require a NW # 3. I have another one here also that you should have for NCSR 1413 (Sandy Cross Road) in Gates County. It is different than the one I already did for NCSR 1413. It is adding two additional crossing to the original request. I'm just going to add the additional crossings as a amendment to the original NW # 3 I did last week.
