Due to the fact that written concurrence is required the review of this project requires a fee to be submitted. The project cannot be reviewed until fees have been submitted. Fees for this project would be in the amount of $240.00 and could be submitted in any of the following ways: 1. If Project is being DOT funded have DOT provide me with the WBS Element code that would allow internal transfer. 2. If Project is not being DOT funded: A. A check made out to NC Division of Water Resources can be mailed to: Transportation Permitting Unit/Wetlands Attn: Kristi Lynn Carpenter 1617 Mail Service Center Raleigh NC 27699-1617 B. A check made out to NC Division of Water Resources can also be hand delivered to the Archdale Building in Raleigh NC located on: 512 N Salisbury Street 12th floor, Room 1219 G Raleigh NC 27604 If you have any questions or concerns regarding this fee please do not hesitate to contact me. Wetlands and Streams – 401 Water Quality Certification information can be found at the following link: https://ncdenr.s3.amazonaws.com/s3fs- public/Environmental%20Assistance%20and%20Customer%20Service/Permit%20Handbook%20Documents/401%20Wat er%20Quality%20Certification.pdf. Also please note that projects submitted on the proper PCN form(s) to both the field staff and myself in the central office can be processed in a more timely manner. Whenever emails are sent with the request just typed in the body of the email it is not always apparent to office staff (namely me) that you are requesting a permit. I understand for this project the Corps did not require the PCN so we have not required the PCN either, however if the PCN were filled out for our program it would make the administrative side of the project much smoother and faster here in the NCDEQ office. We appreciate any accommodations you could make in your office to make the rendering of our services better/faster. Project is currently on hold awaiting your response regarding the proper fee(s). The US Army Corps of Engineers Wilmington District has issued the project the Action Identification Number, SAW-2016- 01685. This correspondence authorizes you to exceed the 500 linear foot maximum stabilization as indicated in Special Condition (f) by the requested 184 linear feet due to the unique nature of the project. If you have not already done so please download a copy of the requested permit at www.saw.usace.army.mil for your file and to be able to review the terms and conditions associated with that permit. Also please be aware that this permit expires on December 31, 2016. 4 If you have any questions please feel free to contact me by email or at the number listed below. From: Williams, Paul C Sent: Thursday, August 04, 2016 3:39 PM To: Barnes, Kyle W SAW; Tracey.L.Wheeler@usace.army.mil; Ward, Garcy Subject: Request for Regional General Permit 80 for Shoreline Stabilization along NC HWY 94 on the Lake Mattamuskeet Causeway in Hyde County NCDOT requests the issuance of a Regional General Permit 80 and corresponding 401 Water Quality Certification for shoreline stabilization along the shoulder of NC HWY 94 on the Lake Mattamuskeet Causeway. The location is just south 6 of Fairfield in Hyde County. Numerous areas of erosion have developed in the last year as a result of wave action impacts that have increased due to Phragmites control along the shoreline of the causeway. The erosion is encroaching on the roadway and the clear recovery area of the roadway. In order to protect the integrity of the roadway and the safety of the clear recovery area, NCDOT proposes to place class B riprap along the shoreline in several sections, as indicated on the included map. The total length of the project would be approximately 684 linear feet, and extend a maximum of 10 feet from the shoreline. Photos of each site are attached. Please let me know if you need any additional information for this project. Thanks, Paul C.
