Dear Mr. Hurley
This is to confirm that a Preconstruction Conference to coordinate construction activities for the above
project will be held on Thursday, October 20th, 2016 at 9:00 a.m. in the Division9 Office (Conference Room
A) located at 375 Silas Creek, Winston Salem, NC 27127.
Your proposed and most current Progress Schedule with Narrative, Letter listing persons authorized to
sign supplemental agreements for your company, Domestic Steel Clause, Erosion Control Certification,
Crane Operators Certification (where applicable), Welder+ Certification (where applicable), Listing of
Subcontractors, Listing of Project Personnel, most current schedule of values, most current table of
quantities, most current payout schedule, your certificate showing the $5,000,000 Liability Insurance,
person authorized to certify DBE affidavits and Material Suppliers list should be submitted as soon as
possible or at least at the Preconstruction Conference.
When using transportation services to meet the contract commitment, the Contractor shall submit a
proposed trucking plan in addition to the SAF. The plan shall be submitted prior to beginning construction
on the project. The plan shall include the names for all trucking firms proposed for use, their certification
type(s), the number of trucks owned by the firm, as well as the individual truck identification numbers and
the item(s) being performed.
Your company representatives both field and management supervisory personnel and subcontractors
should be present at the Preconstruction Conference.
