WWTP NCDWQ General Permit No. NCG030000 Certificate of Coverage NoEGO'f l O r 8" May and June 2016 Stormwater DMRs To Whom It May Concern: I have enclosed two (2) copies of the May and June 2016 Stormwater DMRs for CommScope, Inc. WWTP. The CommScope Certificate of Coverage No. is NCG030078. Please note that results for the May sampling were not received until early July; also, please note that there was no discharge in June, as checked on the DMR form. Please note that (as stated in my previous letter) Mr. George Crosby, EHS Manager, was released by the company and there were several DMRs that were not sent in as a result of these items being found with his belongings (for which I have previously have sent in to your office). Furthermore, I was recently asked to update the Stormwater Pollution Prevention Plan and as a result, I have been trying to catch up with all the issues that were not resolved in an appropriate manner, and as such are now getting back on track with getting all the information sent to DWQ. Also, please note that as of December 7t", 2015, the Signatory Responsibility for the WWTP DMR (NPDES Permit No. NC0034754) was transferred to me. Thus, as a result, I have been onsite as a consultant to assist staff with the wastewater treatment plant operations. However, for the month beginning now (July 2016 forth), the Signatory Responsibility has now been transferred to Mr. Gregory P. Lathrop, whom is the EHS Supervisor and just recently obtained his Grade I Wastewater Operator's License. 1 Anyway, I have enclosed the stormwater discharge report for May 2016 for which samples were collected on May 17th, 2016. Please note that there was no discharge in June (also enclosed). For the sampling conducted in May, CommScope exceeded the bench mark value for zinc (0.067mg/L) for 8 of the 15 sampled outfalls. However, good news is that all other parameters sampled for all 15 outfall samples were in compliance and were below the bench mark values. As for the zinc exceedances, CommScope is working diligently on correcting these Stormwater concerning including updating BMPs and performing qualitative checks to help correct these issues. Furthermore, as noted in the previous correspondence, staff has implemented monthly sampling (along with sampling all outfalls) as per the permit guidance. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray tube manufacture use the definition found in 40 CFR 469.31). **** Please note that Outfall#2 Oil &Grease sample was dropped and broken upon receipt at the laboratory. Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit.
