Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection
conducted on 8/29/2016. The Compliance Evaluation Inspection was conducted by Johnathan C. Watts,
Environmental Specialist, of the Fayetteville Regional Office. The facility was found to be in Compliance
with permit NC0023353. As part of the inspection, a tour of the plant was conducted. All observations
are in Section D. Summary of Findings/Comments of this report. As a reminder, preservation of the
Waters of the State can only be achieved through consistent NPDES Permit compliance.

Enclosed you will find a copy of the Compliance Evaluation Inspection report for the routine inspection
that was conducted on August 29, 2016 by Mark Brantley, Environmental Program Consultant and
Johnathan Watts, Environmental Specialist of the Fayetteville Regional Office. The purpose of the
inspection was to evaluate compliance with the requirements of the Wastewater General Permit
NC0023353. As part of the inspection, a tour of the plant was conducted and weekly monitoring
sample records were examined from March 2016 to April 2016. On the day of the inspection, this
facility was found to be in compliance with the requirements of the permit. You are commended for
your initiative and proactive approach for the documentation and on site monitoring. I appreciate the
time and professional courtesy that you and Timothy Frush, Public Works Director, extended during
the inspection. No additional response is necessary from your office concerning this report. If you

The White Lake WWTP, located at 90 East William St. Elizabethtown NC, produces discharge of
treated effluent from a biological water pollution control treatment system, which consists of a Lagoon
divided into 4 cells. All documentation records and log books appeared to be complete and current with
provisions of the permit. All data appeared to correspond with DMR's. Effluent Flow Meter was
Calibrated on May 26, 2016 and verification of the Chlorine meter calibration curve was performed on
April 1, 2016.
The Vortex Grit Chamber is currently defective and has been out of service since 2006, according
to past inspections. It was determined that as a result of the algae bloom, there was green
discoloration observed in the effluent, however barley straw is used to control growth. As stated from
the previous inspection on August 8, 2013, "During the inspection of the chlorine contact chamber, a
hole was noted in the baffle wall just before the effluent weir. This hole could allow some short
circuiting."
It is strongly recommended that a timeline and plan of action be submitted to perform repairs on
chlorine contact chamber as baffles are critical for the efficient performance in improving the amount of
chlorine contact time with fecal coliforms and possible contamination with pathogens. Restoring the
chlorine contact chamber could reduce operation and maintenance activities and may ultimately
increase reliability for achieving permit compliance. Based on conversations with Mr. Timothy Frush,
progress towards these repairs are being made.
