Dear Mr. Mullis:
On September 13, 2016, Roberto Scheller of this Office conducted an inspection at the subject
facility. This inspection was conducted as a Compliance Evaluation Inspection (CEI) to insure
compliance with permit requirements and conditions. As part of the inspection Toxicity Samples
were collected on September 13th and 15tt'. Analytical results of the toxicity sampling will be
sent under separate cover. Comments on plant operations and deficiency's found during the
inspection are noted on the enclosed inspection report. We wish,to thank you and the operating
staff for assistance regarding this inspection.

should be updated to meet facility's needs Past inspections indicate that sludge is disposed
of by Republic Services to the Uwharrie Environmental Landfill. Please note that Section D,
6. Records Retention,of your current permit, states that sewage sludge records shall be
retained for a period of at least 5 years It is necessary to have past sludge records avalible.
