On August 31, 2016, the Division of Water Resources (Division) received your application
requesting a 401 Water Quality Certification from the Division for the subject project. The
Division has determined that your application is incomplete and cannot be processed. The
application is on -hold until all of the following information is received.-

A review of the Pre -Construction Notification (PCN) Form indicates this application
is for _159_ linear feet of stream impacts. Pursuant to 143-215.3D (e), a $570
permitting fee is required to process this application. Check # 37107 was applied;
however, the remaining balance to remit is $330. Please remit a check to DENR-
DWR, 401 & Buffer Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-
1617.
Pursuant to Title 15A NCAC 02H .0502(e) the applicant shall furnish all of the above
requested information for the proper consideration of the application. If all of the requested
information is not received in writing at the address below within 30 calendar days of
receipt of this letter, the Division will be unable to process the application and it will be
returned. The return of this project will necessitate reapplication to the Division for
approval, including a complete application package and the appropriate fee.
Please be aware that you have no authorization under Section 401 of the Clean Water Act
for this activity and any work done within waters of the state may be a violation of North

Wetland crossings: initial disturbance of these areas shall be limited to only that
area minimally needed to allow passage of construction equipment to prepare for
the installation of the crossing and to execute upland construction on the opposing
side of the crossing. Grubbing or land disturbance of the subject area shall not be
performed until the culverts, backfill material, erosion and sediment control materials
or devices sufficient to substantially complete the construction and stabilization of
the crossing are staged or stockpiled at the location. Due consideration shall be given
to impending inclement weather conditions that may adversely affect the completion
of the crossing in the shortest time practicable.
Drainage work shall be completed in the minimum time necessary.
