REQUEST FOR MORE INFORMATION
Dear Mr. Cebina:
On March 18, 2008 the Division of Water Quality (DWQ) received your application dated
March 18, 2008, to fill or otherwise impact 0.17 acres of wetland, 1791inear feet of perennial
stream, and 615 linear feet of intermittent stream to construct a residential subdivision. The
DWQ has determined that your application was incomplete and/or provided inaccurate
information as discussed below. The DWQ will require additional information in order to
process your application to impact protected wetlands and/or streams on the subject property.
Therefore, unless we receive five copies of the additional information requested below, we will
have to move toward denial of your application as required by 15A NCAC 2H .0506 and will
place this project on hold as incomplete until we receive this additional information. Please
provide the following information so that we may continue to review your project.
Additional Information Requested:
1. Please provide a location map, a copy of the most recent NRCS soil survey map, and a
USGS 1:24,000 topographic map for the project.
2. Please re-submit your site plans on full plan sheets at a scale of no smaller than 1 "=50'
and indicate existing site plan with contours on one map and proposed site plan with
contours on a second map. Additionally, please provide a scale on all submitted maps.
3. Please provide cross section details of all stream-crossing culverts, showing the
provisions for aquatic life passage.
4. Please provide the location and plans and profiles of any proposed stormwater
management practices as required by GC 3705 (and previously GC 3404).
5. Please provide building envelopes for all lots with wetlands and streams on the site plans.
6. Please provide an agent authorization letter for the application signature.

8. 1791inear feet of compensatory mitigation appears to be required for this project. Please
provide a compensatory mitigation plan. The plan must conform to the requirements of
1 SA NCAC 2H .0500 and must be appropriate to the type of impacts proposed.
Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,
