In accordance with Part III, E(2) of the above referenced NPDES Storm water permit, the
original and one copy of the monthly monitoring reports Year 1 - Period 4 are attached. The
period for sampling ended on June 30, 2016.
The outfalls listed on the DMR are "No Flow" for the sampling period. Samples were not
collected from these outfalls due to night time rainfall events or inclement weather events
involving lightning.

In accordance with Part III, E(2) of the above referenced NPDES Storm Water permit, the
original and one copy of the monthly monitoring reports are attached. The samples collected are
required monthly due to Tier Two Status. The period for sampling ended June 30, 2016.
Samples were collected from outfalls on June 15, 2016. Samples for the following outfalls were
collected: SWO16, SW018 and SW020. The laboratory results were received on June 28,
2016 Analytes sampled above the benchmark values for analytical monitoring are as
follows. Duke Energy has performed the steps required by the Tier One and Tier Two status
for these outfalls.

Notes: Exceedances include Copper at SW016, Zinc at SW020'and TSS at SW016, and Mercury at SW016. SW016 failed Hg MS/MSD recovery at 4X dilution at 38/28%
recovery. The sample was then reanalyzed and spiked at lOX dilution with recoveries of 55/80% (acceptable 71-125%). The sample is flagged with an S denoting failing recovery
for MS. PCB and Oil and Grease samples were inadvertently not collected for SW018.
