Per the requirements of the Neuse Riparian Buffer Rule, all stormwater drainage
from this site must be discharged through a correctly designed level spreader.
Current level spreader design requirements are set forth in Chapter 8 of the North
Carolina Stormwater BMP Manual, available at:

2. Please provide one current Level Spreader Supplement Form for each proposed
level spreader, also available at: lrttpaih2o.enr.state.tic.us/suCbmp fonns.h m.
Please include the Required Items checklist and ALL items listed therein (the
submittal will not be approved until all required items are submitted). The DWQ
strongly prefers that this form be completed digitally, printed and submitted. This
allows the designer to use the interactive aspects of this form.
