Enclosed please find our application for renewal of our NPDES Permit and required
documentation.
Aqua North Carolina, Inc. requests the renewal of the current limitations for this facility as
recorded on the previous permit set to expire on December 31, 2016. Aqua is formalizing its
plan to add an equalization basin and begin further upgrades of the existing facility. As part of
these upgrades, Dolphin Bay will be abandoned and flow diverted to the Cape WWTP for
treatment. Aqua has made the request to meet with the Wilmington Regional office to discuss

COASTAL REGION RESIDUALS PLAN
Sludge hauling from the Coastal WWTPs is handled through an agreement with
Lewis Farms and Liquid Waste Inc. During normal operation, waste solids are
wasted to the solids holding tank to be thickened by settling & decanting.
Thickened sludge is tested to determine when it reaches approximately 2% solids
content before hauling. Class B residuals generated by Aqua's Coastal Plants are
picked up Lewis Farms and hauled for a land application as referenced in Permit
No. WQ0000455. In accordance with Section IV Monitoring and Reporting
Requirements of this permit, Lewis Farms is responsible for analyses of
corrosivity, ignitability, reactivity, and toxicity characteristic leaching procedure
(TCLP) at the frequency specified. Lewis Farms shall maintain these results for a
minimum of five years. Attached to this plan is the Operation and Maintenance
Plan as submitted by Lewis Farms and Liquid Waste Inc.
LEWIS FARMS AND LIQUID WASTE, INC.
BIOSOLIDS OPERATIONS AND MAINTENANCE PLAN
I. Purpose
To put into place a plan that implements the requirements of North Carolina Department
of Environment and Natural Resources—Division of Water Quality(NCDENR-DWQ)
Permit#WQ0000455 for Lewis Farms and Liquid Waste, Inc. (Lewis Farms).
Requirements are included for the industrial and municipal biosolid sources (WWTP's)
and Lewis Farms (land application contractor). All biosolid sources are listed within the
permit.
IL Operational Functions
The operational functions of this program involve hauling biosolids from the respective
WWTP's to the land application sites currently permitted under Permit#WQ0000455
and land applying the biosolids in accordance with regulatory requirements. Residuals
will be land applied at or below agronomic rates that are determined using the Realistic
Yield Expectations (RYE) as outlined in the permit. Lewis Farms will also account for
any nutrient sources the landowner/lessee has applied. Some sources will be land applied
as a liquid material utilizing either pumper trucks, vacuum trucks, tractor and pull tank,
and/or a traveling reel system. Some sources will be land applied as a cake material
utilizing a tractor and box spreader. In some instances, biosolids may have to be injected
or surface applied with incorporation depending upon the pathogen and vector attraction
reduction methods employed by the respective WWTP. Whichever method is utilized to
land apply the biosolids, Lewis Farms will spread evenly and at consistent rates.
Biosolids will be land applied on fields that are used for hay production, grazing, or
harvested for animal feed. In the case where biosolids are land applied on fields utilized
for grazing, applications rates will be reduced by 25% in accordance with USDA-NRCS
590 Nutrient Management Standards. Grazed fields will be determined by Lewis Farms
Operations Manager on site.
III. Maintenance Schedules
Lewis Farms will follow the maintenance schedule as recommended by the manufacturer
for the equipment utilized to complete the job within the limits of the permit.
IV. Safety Measures
Lewis Farms takes pride in operating a safety conscious manner. Employees participate
in regular safety training that is designed to help employees recognize safety issues and
take steps to prevent potential accidents from occurring. Lewis Farms employees have
the authority to stop operations if they think any aspect of the operation is going to lead
to the cause of an environmental or personal accident.

V. Spill Response Plan
• Halt the source of the spill: Such as a ruptured line or valve, or damaged transportation
unit.
• Contain spill: Use straw bales to form a barrier. Straw or hay bales can be obtained
from several sources for this purpose. Soil near the site can also be used to form a barrier
for containment.
• Clean up: Use biosolids application equipment to recover as much of the material as
possible. After the application equipment has recovered as much material as possible, it
will be land applied as per the management plan. A temporary sump pit may be dug in
the containment area and spilled material washed into it to enhance recovery.
• Final Clean up: The ultimate goal will be to restore the spill area to its original
condition if possible.

• Management of clean-up efforts: The field superintendent shall take immediate charge
of the clean-up activities. Additional labor will be requested from Lewis Farms as
needed.
VI. Reporting: Within 24 hours of a spill the field superintendent shall present a written report
detailing the cause of the spill and all action taken in response to the spill. Lewis Farms will
notify the Wilmington regional office as soon as possible, but not more than 24 hours after,
or on the next working day following the occurrence
VII. Inspection Plan
Inspections for the land application of biosolids will be conducted at a minimum of a
weekly at the land application site during land application events. Inspections will consist
of inspecting the current field(s) that are being applied on as well as inspecting random
fields that have previously received biosolids applications. Inspections will be performed
by Lewis Farms personnel on a rotational basis to ensure that all employees are familiar
with the inspection processes. At a minimum, these inspections will consist of the
following:

• Verify that a residuals analysis has been collected prior to land application
activities being performed and that it meets the criteria for land application.
• Verify that pathogen and vector attraction reduction data has been collected for the
source to be land applied and that it meets the criteria for land application.
• Verify the crop that is planted or is to be planted on the respective field(s).
• Confirm that the correct amount of biosolids are being land applied.
• Confirm that manned equipment has a copy of the permit and the operations and
maintenance plan.
• Check that the field has been properly flagged and that buffers are adhered to.
• Check that proper signage is posted.
• Confirm that records are turned in at the end of each business day.
Inspections will also be conducted by the respective employees of the biosolids
generating facilities (at the source generating facility). Lewis Farms is not responsible
for inspections at the source generating facilities. These inspections should be
conducted by the respective ORC of each facility and should fall under their own
Operations and Maintenance Plans.
VIII. Sampling and Monitoring Plan
• Sampling for this project consists of sampling the soils of the fields in the respective
permit in order to document that the fields are suitable for land application.
• Biosolids samples are to be collected by the ORC at the respective source generating
facility. This may include nutrient and metal, pathogen, vector attraction reduction,
and TCLP samples. Samples should be provided to Lewis Farms prior to any land
application activities being performed in order to verify that samples meet regulatory
requirements set forth in the permit. Samples should be collected at the frequency
specified in permit #WQ0000455.
• Lewis Farms may perform alkaline stabilization of biosolids at some of the respective
source generating facilities. However, it the responsibility of the ORC at each facility
to provide documentation of times, temperatures, and pH of the biosolids for the lime
stabilization events. Each ORC at the respective source generating facility must be
certified by the NCDEQ-Lab Certification Unit before collecting this data. Lewis
Farms only provides the alkaline stabilization materials and personnel to
perform this operation and is not responsible for the documentation that the
biosotids have passed the alkaline stabilization process.
