Sediment loss reported yesterday morning during inspections at Site 10 and Site 2, project right. ICA report is being issued due to contractor not reinstalling sufficient EC to control site erosion during and after grading operations. At Site 10 area: Slope drain at sta 562, right , was overwhelmed. Storm water flow was high enough at the last drop inlet before Site 10 to overtop the blocking berm beyond the drop inlet. Diversions that had been installed across the roadway were removed during hauling and grading. Rain began while crews were working. Additional silt checks and diversions were not reinstalled before heavy rain started. Slope drains have been installed with inlet protection too high for water to get to the pipe. Crews are onsite cleaning up accumulated silt in the permitted sites. Reset slope drains and install class B rip rap inlet protection - just enough to slow stormwater runoff as it enters pipe. Temp silt ditches along the sides of the roadway need to be installed and silt checks installed in the temp silt ditch. Install silt checks every 50-60' in the temp silt ditches along the sides. Add diversions across the grade E3 8/02/2016 1:39 PM Page 1 of 2 to direct flow into the temp silt ditch and silt checks. This is to channel and slow the flow down before it reaches the slope drains. The drains should be open to allow stormwater to get into them. The silt ditches, checks and diversions should be reinstalled and maintained as work progresses and the Level II Site Supervisor and Forman should review and insure installation. Site 2: Minor silt loss occurred due to storm water overtopping inlet protection on drop inlets. The large silt check has been cleaned and refurbished. Any accumulated silt removed from channel. Direct stormwater runoff to the installed basins at the drop inlets. Cleanout drop inlets and pipe outlet. Discussed getting site runoff into the existing basins. Utilize slope drains to move runoff down slopes around the drop inlet without eroding the slopes. Stabilize the entire area inside the berm around site 2 outlet. Remainder of project: Continue maintenance on EC devices. All "UrgenY' items on the NPDES list need to be addressed. Project will be reinspected by Friday, August 5 for completed corrective actions. 8/02/2016 1:39 PM Page 2 of 2
