Drawer 1578 New Bern, NC 28563 Subject: Preconstruction Conference Minutes Dear Mr. Stevick: The Preconstruction Conference Meeting for the above project was held on July 21, 2016 at the Williamston Resident Engineer's Office. Attached to these minutes is the sign in sheet of who was in attendance. The following items were discussed: Erosion Control • Carla Pugh spoke in regard to the erosion control of the project. She will be monitoring the effectiveness and maintenance of the erosion control measures in place. • Safety Fence and Silt fence should be on the same post. Orange Silt Fence is allowed to satisfy this. • The wattle barrier uses 18 inch wattles. • There is a 90 day ICT to establish vegetation. • Recommended that NPDES paperwork is kept in a notebook format on the project • Mr. Eddie Grant will be in charge of the erosion control and is Leve12 certified. Right Of Way • There was no right of way purchased for this project. Utilities • The utilities owned by MCNC and CenturyLink have been relocated. • The power lines owned by Tideland EMC are expected to be moved to opposite side of pole this or next week according to Ms. Tracey Brown. Questions were raised by the Contractor if this was going to be enough. 15 ft. was stated by Mr. David Stevick to be needed and this would primarily affect Phase 2 of the project. Clay Willis raised the issue that a permit modification would be necessary if poles were moved. Tracey was going to contact Tideland and ask that they not move the power lines at this time until the issue could be resolved. Trader is going to have surveyor to layout furthest pile line to help determine the access requirements for the crane swing in relation to the power poles. Tracey came back to meeting after discussing with Tideland that they could layout an 8 ft. arm for the pole lines to get a 6 ft + offset from the existing poles. Tracey reported that the voltage at this location is 19.9 kV and 34.5 kV phase to phase. Survey Control • Locations and Survey (Mr. Casey Whitley) will prepare a survey control sheet and submit to the RE for distribution to the Contractor. Permits • Mr. Clay Willis opened the discussion of permits and emphasized that the Long Shoal waters were considered High Quality Waters. Each permit was reviewed with the State and Contractor by its respective issuing agency. • Mr. Kyle Barnes reviewed the US Army Corps of Engineers NW 3& 12 permit requirements. • Mr. Garcy Ward reviewed the NCDWQ 401 permit. Specifically mentioned was that the use of nylon mesh grid is not allowed in the erosion control matting. Coir Fiber matting should be used. • Mr. Greg Daisey reviewed the NCDCM CAMA MAJOR permit. • The Contractor is to make himself fully aware of the requirements and conditions of each permit for this project. Contract Review • Date of Availability is September 1, 2016 and the Completion Date is 90 days following acceptance of roadway and structure items. LD's are $500 per day. • ICT #1 availability coincides with the project availability and allows 200 days for construction of the project once work begins. LDS are $500 per day. • Permanent Vegetation Establishment: Once the project has achieved 80% vegetation density and no erodible areas exist within the project limits, the Contractor will be instructed to remove the remaining erosion control items that are no longer needed. • Note to Contractor: Application of epoxy protective coating to tops of all caps and to the exterior faces of the all exterior core slabs units. • Construction Moratorium: No in-water work will be allowed from February 15 through September 30 of any year. • Major Contract Items: None • Specialty Items: Guardrail (#17-20), Long-Life Pavement Markings (#24-25), and Erosion Control (#28-48). Reforestation was also listed but after discussion it will be eliminated. • MBE and WBE requirements of 2.0% and 3.0% utilization each respectfully are expected to be met using Sadler Landscaping, LLC, Hunting Creek Construction, Inc., and R. L. Thompson Co., Inc. • Domestic Steel: Contractor has submitted the "Buy America" Certification however it needs to be notarized and resubmitted. • There is a Twelve Month Guarantee for this project. August l, 2016 Page 3 of 4 • Erosion and Sedimentation Control/Storm water Certification: Contractor has submitted list of names of those who are Level II certified. Contractor's attention directed to the requirements set forth under the NPDES permit. • Contractor expects to utilize Commercial Borrow and Waste sites. Information for sites to be used to be submitted to RE office for approval. • Subletting of Contract: Most SAF forms have been received. Contractor to submit remaining subcontracts as soon as possible. • Clearing and Grubbing is Method II Modified and Grading is Lump Sum. • Bridge Approach Fills: Contractor intends to use stone for select material in construction of approach fills. • Contractor's attention brought to the temperature requirements for placing asphalt on page 36. This may be hard to obtain given the progress schedule anticipated paving timeframes. • AC Base Price is $410.00 per ton. • Guardrail Anchor Units: Contractor`s attention directed to the FHWA acceptance letter and certified working drawings prior to installation. • Steel Sheet Piles was discussed on page 51. The Department had notified Contractor previously that ASTM A572 was preferred. There was some discussion about if the sheet piles came with lifting eyes? Contractor said that they come with lifting eyes. Contractor to verify that the lifting eyes will be located within the concrete cap. • Permanent Seeding and Mulching: There is an incentive for early establishment of vegetation. Note the permit requirement to not use nylon grid erosion control matting. • NPDES documentation must be completed in a timely manner. This is an emphasis area for NCDOT. • There are pay items for floating turbidity curtain and response for erosion control. • Temporary Traffic Control: Project is phased construction with temporary traffic signal. • RE office to initiate three supplemental agreements. SA 1 will be for the Iran Divestment Act. Contractor's attention is directed to completing and submitting the Certification of Eligibility. SA #2 is for Asbestos Assessment. SA#3 is for Concrete Washout Structure. A price is request for both SA #2 and #3 from the Contractor. • Structures Special Provisions: Note provision regarding placing load on decks. This will be especially important after concrete wearing surface is poured. • Construction of Substructure and Construction of Superstructure are both lump sum. • Crane Safety: Contractor will need to submit list of persons who are Competent Persons, Riggers, and Certified Crane operators. • PDA and pile length requirements will be checked on by John Abel. Contract listed quantity is for 1 PDA. The length of piles will affect size of crane to be used and affect previously discussed utility clearance concerns. PLANS On Sheet 2-B, the notes for the Steel Sheet Pile was reviewed. On Sheet 4, the location of the TBM-1 Was noted. Another location was request by RE to Location and Surveys to prevent damage during construction. On Sheet TMP-1A, note 0 is eliminated. Pavement markings will be performed by the Contractor.
