following the reportil�,period. Failure to submit reports as rcquirc(1 will sub_ject the violator to the assessnlcnt of
a civil penalty of up to $25,000 per violation.
To prevent further action, please submit the reports within fifteen (155) days of 1-eceipt of this notice if this
Facility is inactiw 01.not constructed,you may submit a written request to suspend monitoring. 'Che
request should include a begin date and end date for the period of monitoring.
