A review of the November 2015 Discharge Monitoring Report (DMR) for the subject facility revealed the
violation(s) indicated below:

It was also noted that the Compliance Box on the back of the DMR was marked "Compliant"when in fact
it should have been marked "Non-Compliant" with an explanation for the violation.
