A review of the December 2014 self-monitoring report for the subject facility revealed a violation of the
following parameter at Outfall 001:

12/31/14 Flow 0.970 MGD 0.350 MGD (Monthly average)
There have been numerous violations during the past 12 months of the monthly average flow limit at the
Country Woods VWVTP and you have noted on the associated Discharge Monitoring Reports that steps
have been taken to reduce Inflow and Infiltration into the collection system for the Wvv7P. Because the
violations are still continuing this office is requesting a summary of your assessment of the collection
system,the steps already taken to address identified issues,and what actions are still needed to
reduce the volume of wastewater reaching the VWIrTP. Please submit your response to the
attention of Ms. Marcia Allocco using the mailing address noted in the page footer by April 21,
2015.
The Division of Water Resources may pursue enforcement actions for this and any additional violations. If
the violations are of a continuing nature, not related to operation and/or maintenance problems, and you
anticipate remedial construction activities, then you may wish to consider applying for a Special Order by
Consent(SOC).
