A review of the November 2015 Discharge Monitoring Report (DMR) for the subject facility revealed the
violation(s) indicated below:

It was also noted that the Compliance Box on the back of the DMR was marked "Compliant"when in fact

Remedial actions, if not already implemented, should be taken to correct any noted problems. The Division of
Water Resources may pursue enforcement actions for this and any additional violations.
