This project meets the documentation requirements and approval procedures under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project meets the standards of NCDOT's Minimum Criteria Rules. Date: July 27, 2016 Applicant Name: NCDOT Applicant Address: NCDOT Division of Highways P.O. Box 3165 Wilson, NC 27895-3165 Primary Contact for Project: Chad Coggins Phone No.:(2521296-3529 Is this an after-the-fact application: Yes X No River Basin: Neuse Stream Classification: C NSw Regulatory Authorization Options for this Activity Federal: USACE Nationwide General Permit 3— Maintenance State: General Water Quality Certification #3883 and/or Buffer Authorization Local: None Project Description — NCDOT proposes to replace Bridge #83 which is currently a 2 span 36' x 25' bridge with a 1 span 65' x 30' bridge over Juniper Creek. Efforts were made to minimize water quality impacts by containing bridge drainage within the paved shoulder with no deck drains and conveying that water to a grated inlet located beyond buffer zone 2. A rip rap dissipater pad is provided at the outlet of the proposed system to dissipate energy before entering the buffer zone. On the northeastern quadrant, there is a small portion of ditch realignment that will be lined with riprap to slow water down prior to entering buffer zone 2 in order to protect the integrity of the buffer zones. There are 3219 square feet of impacts to Buffer zone 1 and 1342 square feet of impacts in Zone 2 that will be associated with this project. - - Signature: f ' `' Print Name: Chad Coggin Statc of North Carolina IEnvironmental Quality I Water Rcsourccs 167.7 M a1Seni Center, Raih, Noxth Cam.flia 27699 ].617 Phone:91907-(300 Attachment G: Low/Minimal Impact Bridge Project Data Sheet TIP NO Div Managed 2 WBS Element No. 17BP.4.R.74 3 Coun Wilson 4 Brid e Number 83 5 Description Juniper Creek/Bloomery Swamp 6 Basin Neuse 7 Classification C NSW 8 S tre a m S I N 27-86-6-2 9 T e Inland 10 Size 15' 1'I T e 2 s an 12 Existing Structure Size 36'LX25'W 13 Suff. The existing bridge consists of two spans (2 @ 17' 9") with a reinforced (Description of Minimization of Water concrete floor on timberjoists. Interior bents consist of timber caps on timber piles. Abutments are oriented at approximately a 90 degree skew to the roadway. The existing Quality Impacts) bridge utilizes vertical abutments. The replacement bridge will consist of a single span bridge (1 @ 65'-0") with 4'-0" deep caps and vertical abutments with rip rap in front of the abutments. The single span configuration will prevent the need for a interior bent in the creek. Efforts were made to minimize water quality impacts by containing bridge drainage within the paved shoulder with no deck drains and conveying that water to a grated inlet located beyond buffer zone 2. A rip rap dissipator pad is provided at the outlet of the proposed system to dissipate energy before entering the buffer zone. On the northeastern quadrant, there is a small portion of ditch realignment that will be lined with rip rap to slow water down prior to entering buffer zone 2 in order to protect the integrity of the buffer zones. No alterations were made to the existing ditch conveying roadside drainage through buffer zone 2 on the northwest quadrant. Fill slopes will catch at the top of the existing roadway fill and not impact wetlands present within the project limits.
