Page 3 of 10 PCN Form — Version 14 January 2009 PRE- CONSTRUCTION NOTIFICATION (PCN) FORM PROJECT NAME: SAGE MEADOWS PHASE 3 - STREAM CROSSING FOR RESIDENTIAL STREET ATTACHMENT FOR PAGE 3 of 10 ITEMS 3d. and 3e. 3d. - The purpose of this project is to allow for the construction of a new phase of the existing Sage Meadows Residential Subdivision. The new phase will add 59 single-family home lots in response to real estate market demand. Project will incorporate grading and erosion control contracts as well as contracts for utilities installation (electricity, gas, telephone, cable) and street paving. The construction and sale of single-family homes will be the end result of this project. 3e. The project will consist of the installation of 106.6 Lf, of 48" diameter reinforced concrete pipe, two NCDOT approved endwalls and a 35' long x 12' wide x 2' depth rip -rap energy dissipator for the stream crossing. The total impact length is 141.6 Lf. The stream crossing is for street construction in a proposed new phase of the existing subdivision. A small front-end loading machine and track excavator will be used to place the pipe, endwalls and energy dissipator , per plan design, into the proposed impact area of the stream. Either a pump -around system or channel by-pass will be used for stream diversion during construction. Construction time will be one day. Upon completion, all disturbed areas will be cleaned, re -graded per plan specifications and seeded. A project manager, familiar with plan details will oversee the construction process and all employees will be instructed as to the sensitivity of the activity. C. Proposed Impacts Inventory 1.
