Project Information Attach a vicinity map cleazly showing the location of the property with respect to local landmazks such as towns, rivers, and roads. Also provide a detailed site plan showing property boundaries and development plans in relation to surrounding properties. Both the vicinity map and site plan must include a scale and north arrow. The specific footprints of all buildings, impervious surfaces, or other facilities must be included. If possible, the maps and plans should include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the property boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion, so long as the property is cleazly defined. For administrative and distribution purposes, the USACE requires information to be submitted on sheets no lazger than 11 by 17-inch format; however, DWQ may accept paperwork of any size. DWQ prefers full-size construction drawings rather than a sequential sheet version of the full-size plans. If full-size plans are reduced to a small scale such that the final version is illegible, the applicant will be informed that the project has been placed on hold until decipherable maps are provided. 1. Five of the repair sites are located in designated Trout waters (upstream of Nicholson Creek), one repair site is not located in a designated Trout water (downstream of Nicholson Creek). Site 2004-03 consists of approximately 1,1001ineaz feet of the French Broad River located neaz the waste water treatment plant dischazge site for the Town of Rosman. This area is a designated Trout water. Work in this reach will include construction of two structures (rock cross vanes) as well as bank repair to one bank. Adjacent land use on the bank being repaired is agricultural. The opposite bank is a combination of industrial and forested. Work on the opposite bank includes tie-in of the in-stream structures. The U.S. Fish and Wildlife Service (USFWS) reviewed the project location map for this area and requested a survey for the federally Endangered mussel species Appalachian elktoe (Alasmidonta raveneliana). A survey to determine the presence or absence of potentially suitable habitat for this species was conducted on 9 June 2005 under the supervision of Kevin Mazkham. Mr. Mazkham holds the appropriate State and Federal permits to conduct in-stream surveys for this species. Because this project includes in- stream construction, surveys were conducted from a point 100 meters upstream of the project azea to a point 400 meters downstream of the project azea. This reach of the French Broad River was determined to provide limited areas of potential habitat for this species. No individuals of Appalachian elktoe were found in the survey area. The report detailing the survey findings will be forwarded to the USFWS. Site 2004-20 consists of approximately 4201ineaz feet of the French Broad River located approximately 600 feet downstream of Site 2004-03. This site is a designated Trout water. Work in this reach will include the construction of one structure (rock J-hook) as well as bank repair on one bank. Adjacent land use on the bank being repaired includes a combination of agricultural use and residential yard. No work is proposed on the opposite bank. The U.S. Fish and Wildlife Service (USFWS) reviewed the project location map for this area and requested a survey for the federally Endangered mussel species Appalachian elktoe (Alasmidonta raveneliana). A survey to determine the presence or absence of potentially suitable habitat for this species was conducted on 9 June 2005 under the supervision of Kevin Mazkham. Mr. Markham holds the appropriate State and Federal permits to conduct in-stream surveys for this species. Because this project includes in- stream construction, surveys were conducted from a point 100 meters upstream of the project area to a point 400 meters downstream of the project azea. The French Broad River in this reach is a designated Trout water. Work in this reach includes repairing one bank of the unnamed tributary, with additional debris removal in the unnamed tributary and the French Broad River. Adjacent land use consists of residential yards. The U.S. Fish and Wildlife Service (USFWS) reviewed the project location map for this area and requested a survey for the federally Endangered mussel species Appalachian elktce (Alasmidonta raveneliana). A survey to determine the presence or absence of potentially suitable habitat for this species was conducted on 9 June 2005 under the supervision of Kevin Mazkham. Mr. Markham holds the appropriate State and Federal permits to conduct in-stream surveys for this species. Because this project is a debris removal site, the survey was conducted at the debris removal azea.
