A preconstruction conference was held at the Division One Division Office on Thursday, June 23, 2016.
Attendees and Proiect Personnei:
Representing the Prime Contractor, T.A. Loving Company were Clay Wisniewski, Andrew Cestaro, and Stephen
Salter.
Representing the environmental agencies were Tracey Wheeler, USACE, Kyle Barnes, USACE and Garcy Ward,
NCDWR.
Representing NCDOT were Scott Emory, Brooks Braswell, Lance Thomas, John Abel, Andy Blankenship, Nikki
Cobb and Paul Williams.
Scott Emory opened the meeting and asked Tracey Wheeler to review the permit conditions. Garcy Ward
followed by reviewing the DWQ conditions contained in the 401 permit.
Issues:
Mr. Wisniewski stated that work should begin on July 11, 2016 and conclude the first week in October.
Borrow and Waste sites were discussed during the review of the permits. The contractor has provided
documentation from a nearby commercial waste facility. The contractor anticipates sufficient amount of borrow
being generated on the project site, so a borrow pit should not be required.
The old end bents should be removed and grade restored to the elevations required in the plans.
Mr. Emory agreed to make contact with Roanoke EMC to have the power lines dropped to provide a swing radius
for the crane.
Andy Blankenship reviewed several erosion control issues. Carla Pugh will be inspecting the project for
compliance and is scheduled to be on site July 12, 2016.

Concerns were raised over the potential for a waterline in the project limits. Mr. Emory will contact Bertie
County Water Department to verify.
Plan length piles have been ordered with 30' galvanized and 55' steel pile that will be spliced. One pile has been
ordered 5' longer for the PDA test, which will be performed on the first production pile driven.
Mr. Wisniewski asked that a copy of the approved concrete mix design be sent.
Mr. Emory requested that Bullington bring a copy of the guardrail end unit specifications when they come to
install guardrail.
Mr. Braswell agreed to send out the road closure notice for the project on July 11, 2016 at 8:OOam.
Mr. Abel stated that collars under the cap would not be needed due to the use of galvanized pile.
This completes the minutes of the Preconstruction Conference for Contract DA00296 held on Thursday, June 23,
2016 at the Division Office. Any discrepancies with the content of these minutes should be submitted in writing to
this office. Failure to do so will be an indication of agreement.
