Subject: U-5305 Permit Modification request
Andy,
I have attached a copy the original permit application, along with the plan sheet and impact table from the contract for
permit site 1 on U-5305 in Randolph County.
As we had discussed earlier; there is a discrepancy between the requested impacts on the application and the actual
impacts shown on the plans. The plans indicate the installation of a 36"x 36ft long pipe at permit site 1. The PCN and the
impact table shows that only 14ft of permanent impact was requested and approved for this site.
We are formally requesting a permit modification to correct this discrepancy.
