On behalf of The Town of Holly Springs, Kimley-Horn and Associates,
Inc. is submitting the enclosed Construction Sequence (amended), prepared by
Soil and Environmental Consultants, PA, pertaining to the above mentioned
permits for your file. The Construction Sequence has been prepared to
implement compliance measures on the Green Oaks Parkway Project, and the
Town anticipates that construction will commence once the limits of disturbance
have been approved per the Erosion and Sedimentation Control Plan.

Green Oaks Parkway - Construction Sequence Alternates
REVISED - February 12, 2008
The following Construction Sequence Alternates are intended to provide general sequencing
guidance for the remaining headwall construction and proposed stream, wetland and buffer
restoration (repair and stabilization) measures for a series of six culvert crossings along the
Green Oaks Parkway in Holly Springs, NC.
One of two general conditions currently exists at each end (upstream and downstream) of each
of the crossing locations;
A) Headwall Already Constructed & Restoration Remaining, or
Applies to: Crossing #3 Upstream and Downstream
Crossing #5 Upstream Only
Crossing #6 Upstream Only
Crossing #7 Downstream Only
B) Both Headwall Construction & Restoration Remaining
Applies to: Crossing #1 Upstream and Downstream
Crossing #2 Upstream and Downstream
Crossing #5 Downstream Only
Crossing #6 Downstream Only
Crossing #7 Upstream Only
The following general Construction Sequences shall apply at all locations:
Confirm required Limits of Disturbance (LOD) with representatives of the Town of
Holly Springs (TOHS), Wakefield Development, Fowler Contracting, and Soil &
Environmental Consultants, PA (S&EC) (performed on January 28, 2008).
2. Surveyors to re-stake remaining headwall foundations as needed (and directed by
Wakefield). Fowler and Wakefield to confirm headwall staking and coordinate any
modifications in LOD's with S&EC prior to installation of tree protection fencing.
S&EC to notify TOHS of LOD changes.
Fowler to install tree protection fence along newly defined LOD's. Fowler to relocate
(or install new) silt fencing, riprap & gravel checkdams, and other necessary sediment
control devices as deemed necessary.
4. S&EC to inspect installed tree protection fence and sediment control devices for
conforrnance with LOD's and with design intent, and provide additional
recommendations to Fowler as needed.

5. S&EC to provide written confirmation to TOHS that LOD's are in general accordance
with those limits described in the Repair & Stabilization Plan approved by USACE
and DWQ.
Depending on local conditions Sequence A or Sequence B will be followed.
A) If Headwall Already Constructed & Restoration Remaining then:
6A. Fowler to backfill around footings (pump out standing water as needed).
7A. Fowler to perform local stream, wetland and buffer restoration. S&EC to provide on-
site construction observation as required throughout the duration of construction.
S&EC to provide necessary guidance/modifications to repairs based on site conditions
encountered during construction. Fowler to stabilize restoration areas (seeding,
mulching, matting, and planting). S&EC to approve local work upon completion.
8A. Fowler to reinstall local temporary sediment control devices upon completion of local
construction activities. S&EC to notify TOHS and Wakefield of the completion of
local construction activities. Fowler moves to next restoration location.
B) If Both Headwall Construction & Restoration Remaining then:
6B. Fowler to spray paint footer locations and stake out permanent impacts. Fowler (or
their surveyor) to certify that footing and headwall are located as shown on plans.
7B. Fowler to excavate for headwall footings, construct formwork and pour concrete
footings. Remove formwork and backfill footings.
8B. Fowler to perform local stream, wetland and buffer restoration. S&EC to provide on-
site construction observation as required throughout the duration of construction.
S&EC to provide necessary guidance/modifications to repairs based on site conditions
encountered during construction. Fowler to stabilize restoration areas (seeding,
mulching, matting, and planting). S&EC to approve local work upon completion.
9B. Fowler to construct headwall formwork and pour concrete headwall. Fowler to
backfill headwall and stabilize surrounding slopes. Fowler (or their surveyor) to
certify that footing and headwall were constructed as shown on the plans.
IOB. Fowler to reinstall local temporary sediment control devices upon completion of local
construction activities. S&EC to notify TOHS and Wakefield of the completion of
local construction activities. Fowler moves to next restoration location.
NOTES:
1. Construction shall be performed in an upstream to downstream direction at each
crossing location unless otherwise approved by S&EC based on site conditions.

2. S&EC shall provide weekly updates to TOHS and Wakefield outlining the progress of
construction.
3. Planting operations including the installation of livestakes, trees and shrubs may be
delayed until all restoration activities have been completed and weather conditions are
suitable for plant installation. Such delays are in concert with planting efforts described
in the approved Stabilization and Repair Plan.
4. S&EC to prepare Certificate of Completion for restoration efforts for submittal to
USACE and DWQ upon completion of work.
