Dear Sir/Madam.
Smith+Gardner(S+G) is submitting this Year two, Periods 7 through 12 Tier 2, Discharge Monitoring Report
(DMR)for stormwater monitoring at the Waste Industries, USA—Durham Facility. The site is located in Durham,
North Carolina. The facility is permitted to discharge stormwater related to activities that have Vehicle
Maintenance Areas under the NPDES program by the General Permit No. 080000 (permit), issued by the North
Carolina Department of Environment and Natural Resources, Division of Water Quality(DWQ).
For this submittal we are providing you with one (1) copy of the DMR report A full report will be kept onsite which
includes the following items•
1. Stormwater Discharge Outfall(SDO) Report, 2. Qualitative Monitoring forms for the December event;
3 Laboratory data.
As required by the permit,analytical and qualitative monitoring of the discharge will continue The site has
notified the regional supervisor of Tier 3 status. Currently, .S+G has responded to three(3) rounds of comments
received from the City of Durham for the new Basin design to be installed. . It is S+G's understanding is that if all
Durham City/County departments are in agreement with our responses final approval will move forward The
Stormwater Permitting Program will be informed as soon as this approval is granted (expected early 2015).
