On April 19, 2016, the Division of Water Resources (DWR) received a close-out packet from you on
behalf of the Division of Mitigation Services (DMS) requesting approval to close-out the subject site from
monitoring. On May 18, 2016, Katie Merritt with DWR visited the subject site to assess the vegetation
and overall site conditions. On June 14, 2016, Lindsay Crocker with DMS submitted a revised asset map.
The revised asset map is attached.
As a result of Ms. Merritt's review of the close-out request, the following mitigation is approved:
