Please provide a separate BMP Supplement Sheet for the proposed Bioretention Area and Dry Detention Pond (including both worksheets, Design Summary and Required Items Checklist), as well as ALL of the required items. The BMP Supplement Forms are available on the DWQ web site at: http:iih2o.enr.state.nc.us/su~bmp_forms.htm. Some of the required items that were omitted in your submittal include: a. A detailed planting plan for the Bioretention Area. b. A construction sequence. c. Calculations for the underdrain sizing. d. Details of the devices at the appropriate scales. e. Signed and notarized Operation and Maintenance Agreements for each device. 2. Please address the following design issues in your resubmittal: a. The mulch for the Bioretention Area must be triple shredded hardwood. b. The planting media for the Bioretention Area must be a minimum of 36 inches in depth. c. The planting soil mix must be revised to match the requirement in the DWQ BMP Manual for Bioretention Areas. d. The underdrain must be laid at an appropriate slope per the DWQ BMP Manual.
