VAN DER VAART Subject: REQUEST FOR ADDITIONAL INFORMATION Hawthorne at Parkside Dear Mr. Winstead: &Cc 1nry S JAY ZIMMERMAN Dimaor DWR #16-0489 Wake County On May 9, 2016, the Division of Water Resources (Division) received your application dated April 29, 2016 requesting a 401 Water Quality Certification from the Division for your project. The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until the following information is received: 1. The application is incomplete since a number of questions are left blank. The following sections need to be completely filled out: [15A NCAC 02H .0502(b)] • Section A. Applicant Information • Section F.3. Cumulative Impacts 2. In addition, Section E. Stormwater Management and Diffuse Flow Plan contains incomplete and conflicting information. First, there is not a description of the stormwater management plan (SMP). Second, the application states that a SMP is required, but there did not appear to be any stormwater best management practices on the site plan. It is understood that the Town of Cary will be reviewing the SWP, but the Division needs to see at least a conceptual SMP on the site plan. The project area needs to be delineated on the most recent published county soil survey so that the locations of all the mapped streams can be determined and cross referenced with the site plan. [15A NCAC 02H .0502(b)] 4. The plan sheet labelled "Culvert Stream Crossing Profiles" has a note indicating that the total linear feet of stream impact from the culvert crossing is 150 linear feet, but the application states that the total impact length is 140 linear feet. [15A NCAC 02H .0502(b)] Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the proper consideration of the application. If all of the requested information is not received in writing within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it will be returned. The return of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee. Please respond in writing within 30 calendar days of receipt of this letter by sending four copies of all of the above requested information to the 401 & Buffer Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617. Please be aware that you have no authorization under the Water Quality Certification Rules for this activity.
