Comment: Site 10 was reviewed for sediment loss reported on Mav 3. Clean up was underwav. It was determined
that there were too few slope drains installed per the EC plan, which in turn, was a contributinq factor in
the loss. This is in violation of NCDOT's 401 Certification related to erosion and sediment control
practices. A Notice of Violation was issued on Mav 9, 2016 for Site 10. Site 18 was reviewed. Sediment
was deposited near the inlet of the overtlow barrel due to a failure of a sediment control fence. The Site
was repaired satisfactorilv and sediment was removed from the iurisdictional channel.
