HAECO ]Facility Improvements at Piedmont Triad International Airport
Pursuant Special Condition 4 of the subject Section 404 Individual Permit and the Workplan
Verification (enclosed) e-mailed from Michael Baker International on April 25, 2016, the
Piedmont Triad Airport Authority (PTAA, permittee) has verified that the final construction
plans comply with the permit drawings.
Pursuant to Special Condition 5 of the subject Permit, PTAA understand that the Corps have
confirmed compliance with the pre -construction meeting requirement based on the Construction
Schedule dated March 21, 2016 (enclosed) and information provided in the e-mail referenced
above.
Pursuant to Special Condition 9 of the subject Permit, PTAA notified the Corps of proposed
beginning of the authorized work in the e-mail with enclosed construction schedule referenced
above.
Pursuant to Special Condition 12 of the subject Permit, PTAA require contractors and agents to
comply with the terms and conditions of this permit. PTAA have provided contractors
associated with the construction or maintenance of this project with a copy of this Permit which
is also available at the project job site.
Pursuant Special Condition 19 of the subject Permit and the acknowledgements of receipt e-
mailed from Dave Bailey on April 21 and April 27, 2016, PTAA understand that:
• The Corps has received the three (3) Compensatory Mitigation Responsibility Transfer
Forms signed by the Mitigation Sponsors (Restoration Systems, LLC and the NC
Division of Mitigation Services)
• Construction within jurisdictional areas authorized by the permit can begin
• PTAA is no longer responsible for providing the mitigation identified in these forms

• PTAA will retain copies of these executed forms
• PTAA responsibilities specified on these forms have thus been satisfied
Pursuant to Special Condition 20 of the subject Permit, PTAA understand and concur that
214 stream mitigation units (SMUs) of the previously available 702 SMUs from the Causey
Farm mitigation site have been applied to the project authorized under Action
ID SAW -2015-00920, and that no additional SMUs are available at Causey Farm.
Pursuant to Special Condition 21 of the subject Permit, PTAA understand and concur that
1.62 wetland mitigation units (WMUs) of the previously available 3.07 WMUs from Causey
Farm have been applied to the project authorized under Action ID SAW -2015-00920, that these
1.62 SMUs are no longer available to apply to any future project, and that 2.08 WMUs are
currently available at Causey Farm for use on future PTIA projects, pending Corps review.
Pursuant to confirmation from Sue Homewood e-mailed on April 20, 2016, PTAA understand
that the Division WSRO has proof that PTAA payments to the Cripple Creek and Bass Mountain
Mitigation Banks have cleared, that Condition 2 of the subject Section 401 Water Quality
Certification (WQC004054) has thus been satisfied, and that impacts specified in this
Authorization Certificate may occur.

compliance with the subject permit conditions. It is our understanding that PTAA have fully
complied with Special Conditions 4, 5, 9, 12, 19, 20, and 21 of the subject Individual Permit and
Condition 2 of the subject Water Quality Certification.

Dear Mr. Rosser
This letter shall serve as written verification that the final construction drawings for the HAECO
Facility Improvement project conform to the permit plans which are attached to the 404 impact
permit issued by the United States Army Corps of Engineers on April 11, 2016 for final signature.
In the event that there are revisions to the plans, we will notify Mr. David E. Bailey of the Corps of
Engineers Raleigh Regulatory Field Office prior to any active construction in waters or wetlands.
If you have any questions or need any other information regarding this matter, please don't hesitate
