EXPRESS REVIEW ACCEPTANCE LETTER

On January 31, 2008, the 401 OversightBxpress Permitting Unit of the Division of Water Quality received a
request from Mr. James Dasher of S&EC, PA, regarding a project known as "E-27 (Fuquay Elementary)" for
acceptance into the Express Review Program. This letter advises you that your project will be accepted into the
Express Review Program once the following items are received:
1) The application fee of $4,000.00 [for 401 Water Quality Certification with major impacts ($2,000) +
Riparian Buffer Authorization ($2,000)], made payable to the North Carolina Division of Water Quality];
2) Five (5) complete and collated copies of the PCN Application (instead of providing 5 copies of the full size
plans you may provide two copies of the full size plans along with three copies of the 11 X 17);
3) One (1) copy of the completed PCN Addendum
(http://h2o.enr.state.nc.us/ncwetlands/documents/MicrosoftWord-
PCN Addendum 10 Ol 06 OOO.ndf);
4) Five (5) copies of all site plan information pertaining to this project (refer to the PCN Addendum); and
5) Three (3) copies of the BMP supplements and details for on-site diffuse flow provisions (refer to
http: //h2 0. enr. state. nc. us/su/bmp_forms. htm).
Additionally, apre-application meeting (to include the property owner) is required prior to the submittal of your
completed application package. The clock for this project will not start until receipt of the completed application
package and required fee are received. If the impacts are greater than indicated on the Initial Interest Form and/or
the services are different, you may be required to remit an additional fee. If you have any questions or wish to
