Dear Madam,
The North Carolina Department of Transportation (NCDOT) requests the extension of the
Individual 404 Phased Modification and the Individual 401 Water Quality Certification issued for
R-2519A: the improvement of US 19E from SR 1336 (Jacks Creek Road) to SR 1186 (US 19) west
of Micaville in Yancey County. These permits are both set to expire on April 25, 2016. The
following "in water" project construction items still remain to be completed:
• Permit Site 12 - Channel relocation and embankment construction along Little Crabtree
Creek, Station 278+60 to 279+20 +/- Lt.
• Permit Site 14 - Supplemental retaining walls along Little Crabtree Creek, Station
284+40 +/- Lt. (This would exclude the coping work at the top of the walls. All coping
work can be done without being in or impacting the stream.)
• Permit Site 15 - Silt removal at the confluence of Little Crabtree Creek and its UT,
Station 285+00 +/- Lt.

Permit Site 26 - Completion of stream mitigation site on Georges Fork, Station
314+40+/- Lt
Permit Site 28 - Culvert backfill and stream bank work on Little Crabtree Creek, Station
316+40 +/- Lt.
• Permit Site 32 - Steam mitigation repair on UT to Plum Branch, Station 327+60+/- Lt.
NCDOT ensures that these bulleted items will be complete prior to July 1, 2016 and these areas will
also be stabilized by measures such as vegetated or seeded and matted or any other combination of
methods needed to stabilize the soiL If efforts to stabilize the soil are not successful, the�i new
methods and continued efforts to stabilize the area will be required tmtil it is successfiil. There will
be no new impacts associated with the extension of this permit.
NCDOT has construction items that will not be completed by July 1, 2016 but do not involve
working in jurisdictional resources. NCDOT is requesting that the R-2519A Individual 404 Phased
Modification be extended to April 15, 2017 to encompass these iteins but with the caveat that the
bulleted tasks referred to above will be completed prior to July l, 2016.
