FORM: SSGP 10-2013 Page 3 of 3 Attachment to Division of Water Resources Shoreline Stabilization Application Property Owners: Mark and Lisa Perkins 8. Describe proposed work: Shoreline has not been stabilized, and shows signs of erosion with dead or dying trees due to exposed roots. The shoreline measures approximately 325 feet and the slope is very steep. Owners prefer to retain a natural shoreline and tree canopy, and desire to place enhanced rip rap on top of geotextile fabric to deter erosion, and plant native vegetation using live staking or planting of shrubs, reeds, and/or grasses. The proposed work includes placement, by barge, of very large boulders (up to 5-6 feet diameter) at strategic points along the shoreline to support large trees or buffer obvious eroded areas. Smaller boulders will be used to fill in between, leaving areas open for planting native vegetation to retain soil and prevent further erosion. The large boulders/rip rap will extend to about 1 -foot above full lake level. 9. Work will be conducted from water, by barge. Access from land is difficult due to the steep slope and existing vegetation. 10. Total disturbance below normal lake level: No dredging or excavation will be needed below full pond. Boulders will be placed on the lake bottom at the shoreline, and are large enough that no excavated "toe" will be needed to keep them in place. 11. There will be minimal disturbance above normal lake level. Where the steep banks of the shoreline have already slid, or trees have fallen and uprooted earth, those areas will be reshaped to prevent further erosion or soil loss, and supported by rip rap or vegetative plantings. The shoreline is approximately 325 feet, with some minor clearing/reshaping up to about six feet landward, or approximately 1950 square feet. 12. Please see the attached sheet with photos of existing shoreline conditions and notes about trees and vegetation to be removed. 00 -p Ln O 00-0 o tD tv sv O cr sv 0 m O N CD ZZ v (D 0o a m r Co o z z n N to 00 O N _ i I i ! 1 10 H _TT-l IcI iIi I I I I I - -i I o "n 1 w C C r R1 r 6 STATE OF NORTH CAROLINA LINE TABLE GASTON COUNTY LINE I certify that this map was dram from an actual survey made under my LENGTH supervision 10,000'1 The unad)o.t.d error of .1—re as calculated Is V S 88'02'43" E Boundaries not surveyed If any, are noted Deed descriptions are recorded In Book Paye SHOWN E Witness my hand and mail this'I-day of 20 4034 WOT400101101 an yor S 3927'00" E P I PIPE(F / LINE TABLE P C 49 1.O LINE BEARING LENGTH L1 S 88'02'43" E 5080 L2 S 62'23'00" E 4034 L3 S 3927'00" E 3737 L4 S 22'26'00" E 2599 1-5 S 65'03'00" E 3510 L6 S 26'45'00" E 22 27 L7 I S 80'48'00" E 2648 L8 LS 08.53'00" E 1838 L9 FS 13'23'00" W 2475 L10 I S 31'16'00" E 4,379 P I PIPE(F / l/a\GJ 0 ` GRAVEL DRIVE t I 1 ( 1 I 1 I I 0V1OG I WELL i !
