The cooperation of Timothy Lancaster during the site visit was much appreciated. Please advise the staff involved with this NPDES Permit by forwarding a copy of the enclosed report. The main areas of concern that were observed during the inspection: 1. ORC visitation logs have not been maintained. 2. A copy of the NPDES permit must be on-site.
